摘要
目的：探讨腹腔镜下自制套针组合治疗小儿腹股沟斜疝的临床应用价值。方法：回顾分析为257例腹股沟斜疝患儿应用自制套针组合行腹腔镜疝囊高位结扎术的临床资料。结果：257例均顺利完成腹腔镜疝囊高位结扎术,其中行单侧疝手术203例,手术时间8-15 min,平均（10.25±2.12）min;双侧疝手术54例,手术时间18-32 min,平均（25.47±5.62）min;术中出血量1-2 ml,无大出血或血肿。术中发现对侧隐性疝12例,一并结扎处理。患儿均无发热、阴囊水肿、阴囊积气、切口感染等并发症发生,术后1-2 d出院。术后随访3-20个月,平均（11.34±7.98）个月,无疝复发及术侧睾丸发育异常。结论：腹腔镜下应用自制套针组合行疝囊高位结扎术效果确切,具有损伤小、切口隐蔽、康复快、住院时间短、操作简便等优点,符合微创外科要求,适于能开展腹腔镜技术的各级医院,值得临床推广。
Objective： To investigate the clinical value of the self-made needle combination used in laparoscopic treatment for children with indirect inguinal hernia. Methods： The clinical data of 257 children with indirect inguinal hernia who underwent laparoscopic high ligation of hernia sac with self-made needle combination were retrospectively analyzed. Results： All the 257 cases were successfully completed laparoscopic high ligation of hernia sac,including 203 cases of unilateral hernia surgery and 54 cases of bilateral hernia surgery,the operation time was 8-15 min with the average of（ 10. 25 ± 2. 12） min for unilateral hernia,and 18-32 min with the average of（ 25. 47 ± 5. 62） min for bilateral hernia. The blood loss was 1-2 ml,there was no hemorrhage or hematoma. During surgery,12 cases of contralateral recessive hernia were found and ligated. After surgery,children were discharged after 1-2 d. There were no fever,scrotal edema,scrotal pneumatosis,incision infection or other complications. Patients were followed up for 3-20 months with an average of（ 11. 34 ± 7. 98） months,no hernia recurrence or operative side testicular abnormalities occurred. Conclusions： Laparoscopic high ligation of hernia sac with self-made needle combination is effective,has advantages of mini-invasion,hidden incision,quick recovery and short hospitalization time,simple operation and so on. This procedure conforms to the requirements of the minimally invasive surgery,is able to be carried out in all levels of hospitals with laparoscopic technology,is worth the clinical promotion.
