线性探针技术在新疆耐药结核病诊断中的应用评价 被引量:2

Evaluation on linear probe assays in diagnosis of drug-resistant tuberculosis in Xinjiang

摘要 目的评估线性探针技术在快速检测耐药结核病中的临床应用效果。方法收集2016—2017年在新疆维吾尔自治区胸科医院就诊的789例结核病病人的痰培养阳性标本,进行传统药物敏感检测,同时开展线性探针检测技术复核,分析线性探针技术检测结核杆菌对异烟肼和利福平耐药的灵敏度及特异度。结果以传统药物敏感检测技术为金标准,线性探针技术检测异烟肼耐药的灵敏度为85.42%(123/144)、特异度98.76%(637/645)、阳性预测值93.89%(123/131)和阴性预测值96.81%(637/656),检测利福平耐药的灵敏度83.92%(120/143)、特异度98.45%(636/646)、阳性预测值92.31%(120/130)和阴性预测值96.51%(636/659)。结论线性探针技术能够快速检测异烟肼和利福平的药物敏感性,提高了耐药结核病的筛查和诊断,为调整治疗方案提供了可靠的实验室依据。 Objective To evaluate the effect of clinical application of linear probe in rapid detection of drug-resistant tuberculosis. Methods The sputum samples with positive sputum culture were collected from 789 tuberculosis patients in Xinjiang Chest Hospital for traditional drug sensitivity test and linear probe review from 2016 to 2017. The sensitivity and specificity of linear probe test were analyzed in the detection for resistance of Mycobacterium tuberculosis to isoniazid and rifampicin. Results Under the gold standard of traditional drug sensitive test, the sensitivity, specificity, positive predictive value and negative predictive value of linear probe test were 85.42% (123/144), 98.76% (637/645), 93.89% (123/131) and 96.81% (637/656), respectively, in the resistance of Mycobacterium tuberculosis to isoniazid, and 83.92% (120/143), 98.45% (636/646), 92.31% (120/130) and 96.51% (636/659), respectively, in the resistance of Mycobacterium tuberculosis to rifampicin. Conclusions The drug sensitivity to isoniazid and rifampicin can be quickly detected by linear probe test, the effect of screening and diagnosis of drug-resistant tuberculosis is improved, and reliable laboratory evidence is provided for the adjustment of treatment plan.
