摘要
儿童手术以"短、平、快"手术居多,特别适合开展日间手术,传统的儿童住院手术正逐渐转为日间手术.本文对我国儿童日间手术的纳入标准、围术期管理、手术管理、规范和流程等进行了综述.我国日间手术开展相对较晚,目前对于儿童日间手术没有统一标准或指南,同样面临医疗安全问题,建立健全医疗质量与安全管理的相关制度、流程至关重要.与此同时,将加速康复外科(ERAS)理念引入儿童日间手术领域,可有效减少外科应激反应,加速患儿术后快速康复.由于儿童生理及心理发育的固有特点,使儿童与成人日间手术围术期管理存在着差异:从术前宣教、术前准备、手术及麻醉、疼痛管理、出院管理、居家康复指导等均围绕儿童的特点,积极探索一套具有儿童特色的日间手术管理模式.
As most Pediatric surgeries need short time and stable condition,which are particularly suitable for day surgeries,pediatric surgeries in traditional hospitalization have gradually turned to day surgeries.This paper reviews the inclusion criteria,perioperative management,surgical management,norms and procedures of day surgeries for children in China.Day surgeries developed comparatively late in China,and there are no uniform standards or guidelines for pediatric day surgeries at present.Pediatric day operations also face the medical security problems,so it is of vital importance to establish and improve the relevant systems and procedures of medical quality and safety management.At the same time,the concept of accelerated rehabilitation surgery (ERAS)was introduced into the field of pediatric daytime surgery,which can effectively reduce the surgical stress response and accelerate the postoperative rapid recovery of children.There is a difference between the management of day surgery in children and adults because of children's physiological and psychological characteristics.A set of day surgery management model are explored based on the characteristics of children exists in the preoperative education,preoperative preparation,surgery and anesthesia,pain management,management of hospital discharge, home rehabilitation guidance,etc.
