大鼠脊髓切断术后神经纤维残留量与运动功能恢复的关系 被引量:20

摘要 目的 观察成年大鼠下胸段脊髓切断术后是否出现神经纤维残留 ,以及神经纤维残留量与后肢运动功能恢复的关系。 方法 取 12只成年大鼠 ,以尖刀片沿椎管骨壁横切下胸段脊髓。术后定期观察截瘫后肢运动功能恢复情况 ,8周处死动物 ,灌注后取损伤部位及其上下节段脊髓 ,矢状冰冻切片 ,抗神经丝抗体免疫组织化学染色 ,光镜下观察有无残留神经纤维 ,计算机技术重建脊髓横断面 ,确定残留纤维的部位及其所占脊髓横断面的比例 ,并分析残留纤维数量与后肢运动功能恢复程度的相关性。 结果  42 %动物出现程度不等的后肢运动功能恢复 ,其脊髓横切处腹侧或腹外侧有数量不等的神经丝免疫反应残留纤维 ,并与后肢运动功能的恢复呈正相关关系。 结论 以尖刀片横切大鼠脊髓的常用方法 ,易造成脊髓神经纤维的残留 ,少量残留纤维即可引起后肢运动功能相当程度的恢复。因此 ,在评价脊髓损伤后的功能恢复时 ,必须首先确定损伤是否完全。 Objective The present study was aimed at investigating the possibility of remaining spinal fibers and the relationship between such remaining fibers and the functional recovery of locomotion in the hindlimbs following spinal cord transection in adult rats. Methods The lower thoracic spinal cords of 12 adult rats were transected with a sharp blade along the inner wall of the vertebral canal. The functional recovery was evaluated and recorded regularly after surgery in each of the animals. Animals were slaugtered after a survival period of 8 weeks. The damaged segments of the spinal cords were sectioned sagittally and prepared for anti-neurofilement immunostaining as well as image analysis. Results 42% animals showed different degrees of functional recovery, which correlated well with the amount of remaining spinal fibers passing through the lesion site in the ventral or ventrolateral part of the cord. Conclusion Conventional spinal cord transection with sharp blade often leads to the remaining of the spinal fibers in the ventral or ventrolateral part of the cord and sequential functional recovery of the spinal cord. In view of this, the completion of spinal cord transection must be assured before any function recovery can be assessed.;
