摘要
目的:比较锁骨钩钢板与双Endobutton带袢钢板治疗TossyⅢ型肩锁关节脱位的临床疗效与影像学表现。方法:回顾性分析我院骨伤科自2011年1月-2013年4月收治的、随访资料齐全的47例TossyⅢ型肩锁关节脱位患者资料,其中30例行锁骨钩钢板固定,17例行双Endobutton带袢钢板治疗,比较两者的手术时间、临床疗效、术后X线表现及治疗费用。结果:两组在手术时间上比较差异无统计学意义(P>0.05);术后平均随访6个月,最终随访时带袢钢板组肩关节Constant评分稍高于锁骨钩钢板组,但差异无统计学意义(P>0.05);术后X线显示带袢钢板组锁骨远端均解剖复位,而钩钢板组2例锁骨远端低于肩峰端、1例钩钢板钩端偏长;治疗费用对比差异无统计学意义(钩钢板组取内固定费用已计入)。结论:治疗TossyⅢ型肩锁关节脱位,双Endobutton带袢钢板在复位程度、防止并发症方面优于锁骨钩钢板。
Objective：To compare the clavicular hook plate with double Endobutton plate with cerclage in curative effect and imaging findings in the treatment of Tossy type Ⅲ acromioclavicular joint dislocation. Methods：A ccording to retrospective analysis of 47 cases of Tossy type Ⅲ acromioclavicular joint dislocation admitted to the department of orthopedics and trau-matology of our hospital from 2011 January to 2013 April with complete followed up data, including 30 cases of clavicular hook plate fixation, 17 cases of double Endobutton plate with cerclage fixation , and the operation time,curative effect, postoperative X- manifestations and treatment cost were compared between two groups. Results：There was no significant difference （P〉0.05） in the operation time in the two groups ; A mean 6 months follow-up after surgery, finally shoulder joint Constant score of the plate with eerclage group slightly higher than the clavicular hook plate group, but there was no statistical difference between them （P〉0.05） ; Anatomical reduction of distal clavicle was got in plate with cerclage group displaied from post-operation X-ray, while in 2 cases of hook plate group the distal part of clavicle was lower than the acro-mial end, and in 1 case the hook side was too long; There was no significant statistical difference in the cost of treatment （the cost of remove the fixtion had already been counted）. Conclusion：In the treatment of Tossy type Ⅲ acromioclavicular joint dislocation, double Endobutton plate with cerclage is superior to the clavicular hook plate in the reduction extent, complications preventing.
