摘要
目的探讨不同分娩方式对产后早期盆底肌功能的影响情况。方法选取2011年9月-2015年5月郑州大学第三附属医院门诊产后6~8周复查的产妇5057例。根据不同分娩方式分为选择性剖宫产组(1495例)和阴道分娩组(1564例),分别通过盆底功能检测仪检测肌力、疲劳度、阴道动态压力。结果选择性剖宫产组Ⅰ类肌纤维肌力受损1464例(981%),Ⅱ类肌纤维肌力受损1471例(986%),阴道分娩组Ⅰ类肌纤维肌力受损I555例(993%),Ⅱ类肌纤维肌力受损1556例(99.5%),两组比较差异有统计学意义(P〈0.05)。选择性剖宫产组Ⅰ类和Ⅱ类肌纤维疲劳度,阴道动态压力分别(1.96±0.061)、(145±0066)和(29.06±0.87)与阴道分娩组(1.75±0.054)、(125±0.060)和(26.56±0.59)比较,差异有统计学意义(P〈0.05)。结论相对于选择性剖宫产,阴道分娩对产后早期盆底肌的损伤更明显。
Objective Effect of different delivery methods on the pelvic floor muscle dysfunction in the early postpartum.Methods From September in 2012 to may in 2013,3057 women after delivered 6-8 weeks were selected and divided into two groups according to different delivery methods, one was selective cesarean section delivery group (1493 cases) and the other was vaginal natural delivery group (1564 case) Test pelvic floor muscle strength,fatigue,strength and pelvic floor pressure by equipment to explore pelvic floor function. Results Type I muscle fibers damaged of the Selective cesarean section group has 1464 cases (98.1%) Type H muscle fibers damaged has 1,471 cases (98.6%):type I muscle fibers of the vaginal natural delivery group has 1,553 cases (99.3%) Type II muscle fibers damaged has (99.5%).type I and type II muscle fibers'fatigue,pelvic floor pressure by equipment to explore pelvic floor function of the selective cesarean section delivery group was (1.96~0.061 ), ( 1.43~0.066), (29.06+0.87) respectively, further ,the vaginal natural delivery group was (1.73~0.054), (1.25~0.060), (26.56+0.39) respectively.There was significant difference between two groups (P〈0.05).Conclusion Two delivery methods both impact on early postpartum pelvic floor muscle strength, damage of the vaginal natural delivery was more serious than the cesarean section delivery.
