摘要
目的盆底功能障碍临床治疗以物理疗法为主,多采取盆底肌锻炼,但效果并不理想;生物反馈电刺激疗法有利于改善盆底功能障碍。本研究探索生物反馈电刺激疗法配合盆底肌锻炼对妇女产后盆底功能康复影响。方法选取2016-10-03-2018-10-20于南阳市第三人民医院进行分娩且产后6周诊断为盆底功能障碍的120例产妇作为研究对象,按照组间基本特征均衡可比的原则分为观察组和对照组,各60例。观察组采用生物反馈电刺激疗法配合盆底肌锻炼的康复训练治疗,对照组采用盆底肌锻炼康复训练治疗。治疗3个月后对比两组产妇治疗前后盆腔脏器脱垂和尿失禁情况、两种盆底肌肌力变化、盆底肌Dmax和盆底肌持续收缩60s的H值。结果观察组盆腔脏器脱垂治疗总有效率为95.00%,高于对照组的76.67%,χ2=8.291,P=0.004。观察组尿失禁治疗总有效率为96.67%,高于对照组的75.00%,χ2=11.582,P<0.001。治疗后观察组Ⅰ类肌纤维肌力为(4.13±1.15)级,高于对照组的(2.83±1.06)级,t=6.438,P<0.001;Ⅱ类肌纤维肌力为(3.57±1.52)级,高于对照组的(2.55±0.62)级,t=4.813,P<0.001。治疗后观察组Dmax为(25.12±3.21)μV,高于对照组的(18.02±2.11)μV,t=14.317,P<0.001;观察组H值为(17.24±2.49)μV,高于对照组的(13.87±3.14)μV,t=6.514,P<0.001。结论生物反馈电刺激疗法配合盆底肌锻炼可提高盆底肌纤维肌力,有效改善妇女产后盆腔脏器脱垂和尿失禁情况,有助于盆底功能恢复。
OBJECTIVE The clinical treatment of pelvic floor dysfunction is mainly physical therapy,and more pelvic floor muscle exercise is taken,but the effect is not ideal;biofeedback electrical stimulation therapy is beneficial to improve pelvic floor dysfunction.This study explores the effect of biofeedback electrical stimulation therapy combined with pelvic floor muscle exercise on women’s postpartum pelvic floor functional rehabilitation.METHODS A total of 120 cases of postpartum women diagnosed as pelvic floor dysfunction in Nanyang Third People’s Hospital were selected from October 3,2016 to October 20,2018 for 6 weeks after delivery.The observation group received biofeedback electrical stimulation therapy combined with pelvic floor muscle exercise rehabilitation training,while the control group received pelvic floor muscle exercise rehabilitation training.After 3 months of treatment,the pelvic organ prolapse and urinary incontinence of the two groups of women before and after treatment,the changes of the two pelvic floor muscle strength,the Dmaxof the pelvic floor muscle and the H value of the pelvic floor muscle continued to contract for 60 swere compared.RESULTS The total effective rate of pelvic organ prolapse in the observation group was 95.00%,which was higher than that of the control group 76.67%,χ2=8.291,P=0.004.The total effective rate of urinary incontinence in the observation group was 96.67%,which was higher than that of the control group 75.00%,χ2=11.582,P<0.001.After treatment,the observation groupⅠmuscle fiber strength was(4.13±1.15)grade,which was higher than that of the control group(2.83±1.06)grade,t=6.438,P<0.001;the typeⅡmuscle fiber was strength(3.57±1.52)grade,which was higher than that of the control group(2.55±0.62)grade,t=4.813,P<0.001.The Dmaxof the observation group after treatment was(25.12±3.21)μV,which was higher than that of the control group(18.02±2.11)μV,t=14.317,P<0.001;the H value of the observation group was(17.24±2.49)μV,which was higher than that of the control group(13.87±3.14)μV,t=6.514,P<0.001.CONCLUSION Biofeedback electrostimulation combined with pelvic floor muscle exercise could improve pelvic floor muscle strength,effectively improve pelvic organ prolapse and urinary incontinence in women,and contribute to the recovery of pelvic floor function.
