摘要 目的探讨饮食因素在肠易激综合征(irritable bowel syndrome,IBS)患者中的作用。方法对海军总医院消化内科2010年1月—2013年1月因腹部症状就诊的患者和健康体检中心的体检者采用自行设计的依据罗马ⅢIBS诊断标准并结合IBS相关饮食因素的调查问卷进行问卷调查,将调查问卷合格的IBS 3242例设为观察组、健康体检者1520例设为对照组。对两组饮食因素进行比较,并对产生的影响因素进行Logistic分析。结果两组进食辛辣食物频率、进食乳制品频率、进食生冷食物频率、进食粗粮或高纤维食物频率、进食高脂食物频率、进餐时间、每日饮水量差异有统计学意义(P<0.05)。Logistic分析结果显示,高频率进食乳制品、生冷食物、高脂食物及每日饮水量过大均为IBS患病的危险因素(P<0.05,P<0.01),而进食时间长、高频率进食粗粮或高纤维食物为IBS的保护因素(P<0.01)。结论良好的饮食习惯及合理的饮食结构对于控制IBS症状和发病具有一定的意义。 Objective To explore the effect of dietary factors in patients with irritable bowel syndrome (IBS). Methods Patients visited doctors for abdominal symptoms in gastroenterology department and healthy persons with medi- cal examinations during January 2010 and January 2013 filled in the questionnaires designed on Rome m diagnostic criteria for IBS and combined with eating habits related to IBS. A total of 3242 patients with IBS, who were qualified to fill in the questionnaire, were chosen as observation group, while 1520 healthy controls were chosen as control group. Eating habits in the two groups were compared, and the influencing factors were examined by the logistic analysis. Re- sults The differences in the frequencies of eating spicy food, dairy products, uncooked or cold foods, coarse grain food or high-fiber foods and high-fat food, meal times and volumes of daily water intaking were statistically significant (P 〈 0.05). Logistic regression analysis showed that the risk factors of IBS were high frequencies of eating dairy products, un- cooked or cold foods and high-fat food and excessive daily water intakirlg (P 〈 0.05 ,P 〈 0.01 ) , while the protective fac- tors of IBS were slow eating and high frequency of eating coarse grain food or high-fiber food (P 〈 0.01 ). Conclusion Good dietary factors and reasonable dietary structures are helpful for controlling the symptoms and reducing the IBS inci- dence rate.
