摘要
[目的]探讨补阳还五汤联合鼠神经生长因子对前部缺血性视神经病变(anterior ischemic optic neuropathy,AION)视功能减退的临床治疗价值。[方法]将156例AION患者随机分为对照组(35例)、补阳还五汤治疗组(42例,以下简称为中药组)、鼠神经生长因子治疗组(38例,以下简称为西药组)及补阳还五汤联合鼠神经生长因子治疗组(41例,以下简称为联合组),记录治疗开始前和治疗开始后1周、1个月、3个月时所有入选者的视野、最佳矫正视力。[结果]治疗开始后1周,联合组患者的视野平均偏差、视力提高排数及总有效率与对照组比较,差异有统计学意义(P<0.05);但与中药组、西药组比较差异无统计学意义(P>0.05);对照组、中药组、西药组两两比较差异均无统计学意义(P>0.05)。治疗开始后1个月,联合组患者的视野平均偏差、视力提高排数及总有效率与对照组比较,差异有统计学意义(P<0.01),但与中药组、西药组比较无统计学差异(P>0.05);对照组、中药组、西药组两两比较差异均无统计学意义(P>0.05)。治疗后3个月,联合组患者的视野平均偏差、视力提高排数及总有效率与对照组、中药组、西药组分别比较差异均有统计学意义(P<0.01);对照组与中药组、西药组比较差异也有统计学意义(P<0.05);但中药组与西药组比较差异无统计学意义(P>0.05)。[结论]补阳还五汤联合鼠神经生长因子治疗AION不仅在病程早期即能改善视功能减退,且治疗时间越久,疗效越明显,从中长期(3个月)来看,对视功能减退的改善均优于基础治疗、单独补阳还五汤及单独鼠神经鼠生长因子治疗。
[Objective]To discuss the clinical value of the treatment on visual hypofunction of the patients with anterior ischemic optic neuropathy by buyang huanwu decoction combined with mouse nerve growth factor. [Methods]One hundred and fifty-six patients who were diagnosed as anterior is-chemic optic neuropathy were randomly divided into four groups which were control group（35 eyes）,buyang huanwu decoction treatment group（42 eyes, traditional Chinese medicine group）,mouse nerve growth factor treatment group（38 eyes,western medicine group） and buyang huanwu decoction com-bined with mouse nerve growth factor treatment group（41 eyes,combined group）.We recorded the results of visual field and the best corrected visual acuity before treatment,a week after the beginning of the treatment,a month after the the beginning of the therapy（P〈0.05）.But there were no statistical differ-ences among the other groups（P〉0.05） on this time node. There was statistical difference between control group and combined group about the visual field,vision improvement and total effective rate a week after the beginning of the therapy（ P〈0.01）.But there were no statistical differences among the other groups（P〉0.05） on this time node. At the time of three months after the beginning of the therapy,there was statistical difference between control group and the other three groups about the visual field,vision improvement and total effective rate（P〈0.05）,but was no statistical difference between tradi-tional Chinese medicine group and western medicine group（ P〉0.05）.And the control group was different from the traditional Chinese medicine group and western medicine group statistical y. [Conclusion]It is helpful to the patients who are diagnosed as anterior ischemic optic neuropathy by treatment of buyang huanwu decoction combined with mouse nerve growth factor on the early stage.The visual hypofunction can be relieved significantly by the com-bined therapy from a medium and long term（3 months） view.The combined treatment is not only better than basic treatment but also better than tradi-tional Chinese medicine treatment or western medicine treatment.
