摘要
目的 探讨聚焦解决模式在妊娠期糖尿病患者孕期随访管理中的应用及对妊娠结局的影响.方法 选择孕期检查并住院分娩的妊娠期糖尿病患者180例,根据患者在孕期随访管理中是否应用聚焦解决模式进行临床干预分为观察组及对照组各90例,比较两组患者妊娠期并发症发生率及围产儿并发症发生率.结果 两组剖宫产率比较,差异无统计学意义（x^2=1.283 2,P＞0.05）；观察组患者围产期并发症发生率中妊娠期高血压疾病、胎膜早破、羊水过多、产后出血的发生率分别为15.55％,12.22％,14.44％,2.22％,均低于对照组的42.22％，30.00％，32.22％，12.22％,差异均有统计学意义（x^2分别为15.576 9,8.539 7,7.950 3,6.715 8;P ＜0.01）；观察组围产儿并发症发生率如早产儿、巨大儿、胎儿窘迫、新生儿窒息、新生儿低血糖反应的发生率分别为17.78％，11.11％，5.50％，1.11％，6.67％，均低于对照组的47.78％,36.67％,11.11％,16.67％,21.4％,差异均有统计学意义（x^2分别为18.5340,16.163 6,7.456 8,13.445 1,7.850 3;P＜0.01）.结论 应用聚焦解决模式对妊娠期糖尿病患者进行孕期随访管理,可降低围产期并发症及围产儿并发症发生率,具有重要的临床意义.
Objective To study the application and effect of solution focused approach on patients with GDM during pregnancy follow-up.Methods Totals of 180 patients with GDM who chose pregnancy examination and hospital delivery were chosen,and divided into the experimental group and the control group according to whether using solution focused approach during follow-up or not,each with 90 cases.Incidence rates of rinatal complication as well perinatal complication were compared.Results There was no statistically significant difference of cesarean section rate between two groups （x^2 =1.283 2,P 〉 0.05）.The incidence rate of rinatal complication was respectively 15.55% for hypertensive disease,12.22% for premature rupture of fetal membranes,14.44% for polyhydramnios and 2.22% for postpartum hemorrhage in the experimental group,all lower than those in the control group （respectively 42.22%,30.00%,32.22% and 12.22%）,and the differences were statistically significant （x^2 =15.576 9,8.539 7,7.950 3,6.715 8,respectively; P 〈 0.01）.The incidence rate of perinatal complication was respectively 17.78% for premature infant,11.11% for giant baby,5.50% for intrauterine asphyxia,1.11% for asphyxia neonatorum and 6.67% for hypoglycemia in the experimental group,all lower than those in the control group （respectively 47.78%,36.67%,11.11%,16.67% and 21.4%）,and the differences were statistically significant （x^2 =18.534 0,16.163 6,7.456 8,13.445 1,7.850 3,respectively; P 〈 0.01）.Conclusions Solution focused approach has important clinical significance for pregnancy follow-up management of patients with GDM,as well as reducing incidence rates of rinatal and perinatal complication.
