Targeted surveillance of nosocomial infections in NICU

摘要 目的调查新生儿重症监护病房(NICU)医院感染的发病情况及相关危险因素。方法按照卫生部制定的《医院感染监测规范》中新生儿病房医院感染监测方法监测2010年1月-2011年12月入住NICU患儿的感染率,并统计分析。结果共监测NICU患儿2966例,发生医院感染59例、69例次,医院感染率1.99%、例次感染率2.32%、日感染发病率4.01‰,感染率与出生体重密切相关;感染部位以血液系统、胃肠道及下呼吸道感染为主,分别占24.64%、21.74%、20.29%;PICC置管使用率和相关血流感染率为5.24%和1.16‰,呼吸机使用率和呼吸机相关性肺炎发生率为13.45%和3.61‰;检出病原菌38株,以肺炎克雷伯菌多见,占42.12%。结论通过目标性监测有利于分析医院感染的高危因素,制定有效的医院感染的预防与控制措施。 OBJECTIVE To investigate the incidence and risk factors of nosocomial infections in a neonatal intensive care unit ( NICU ). METHODS The targeted surveillance of nosocomial infection in a neonatal intensive care unit was carried out according to the method involved in the Nosocomial Infection Surveillance System developed by Mininstry of Health, P.R. China. The statistical analysis was performed for the infection rate of the neonates who were enrolled the NICU from Jan 2010 to Dee 2011. RESULTS Of totally 2966 neonates under the surveil-lance, 59 cases were infected with 69 case times, the nosocomial infection rate was 1.99%, the case-time infection rate was 2. 32M, and the daily infection rate was 4. 01‰, the infection rate was closely related to the birth weight. The blood system, gastrointestinal tract, and lower respiratory tract were the predominant infection sites, accounting for 24. 64%, 21. 74%, and 20. 29%, respectively. The utilization rate of PICC was 5. 24%, the incidence rate of PICC-related bloodstream infections was 1.16‰, the utilization rate of ventilator was 13.45%, and the incidence rate of ventilator-associated pneumonia was 3. 61‰. Totally 38 strains of pathogens were isolated, among which Klebsiella pneumoniae was dominant, accounting for 42.12%. CONCLUSION It is condu- cive to analyze the high-risk factors of nosoeomial infections and to put forward effective prevention and control measures through the targeted surveillance.
