薏苡仁在减轻老年食管癌肺癌联合放化疗中毒副反应的作用

Observation of the effects of coix seed extract combined with chemoradiotherapic treatment of the elderly esophageal or lung malignant tumors

摘要 目的:观察薏苡仁提取物加放、化疗治疗老年食管癌、肺癌的治疗效果及与使用剂量的关系。方法:经手术或病理确诊老年食管癌、肺癌84例患者,随机分为两组:(1)观察组41例,在放疗(化疗)治疗基础上联合应用薏苡仁提取物,100mL或200mL静滴,1次/日,连续2周,停1周,每3周为1个周期,共2周期。(2)对照组43例使用放疗(化疗),同时予以5%葡萄糖500mL加维生素C等静滴,1次/日,连续2周,停1周后重复;两组患者采用常规分割放射治疗。结果:观察组近期总有效25例(60.9%),对照组近期总有效22例(51.2%),两组比较差异无统计学意义(P>0.05)。两组胃肠道反应、骨髓抑制、放射性肺炎、放射性食管炎行相关分析,差异有统计学意义(P<0.05);观察组薏苡仁提取物剂量为100mL/d和200mL/d者KPS评分变化分别为22例(84.6%)和13例(86.7%),体重变化分别为23例(92.0%)和14例(93.3%),较对照组24例(55.8%)和29例(67.4%),差异有统计学意义(P<0.05)。使用100mL/d与200mL/d剂量者KPS评分或体重变化比较差异无统计学意义(P>0.05()可信区间包含0)。放疗计划完成率观察组为92.7%,对照组为81.4%,两组比较差异无统计学意义(P>0.05);化疗计划完成率观察组为90.2%,对照组为72.1%,两组比较差异有统计学意义(P<0.05)。毒副作用引起的治疗间断时间观察组为1.35±0.27天,对照组为3.32±1.04天,两组比较差异有统计学意义(P<0.05)。结论:薏苡仁提取物可减轻放、化疗引起毒副反应,放、化疗不良反应严重程度随薏苡仁提取物剂量增加而下降。 Objective:To observe the clinical toxicity and efficacy of coix seed extract combined with chemoradiotherapy in the treatment of the elderly esophageal or lung tumors and the relevant factors with its efficacy. Methods: 84 cases diagnosed as esophageal or lung malignant tumors after surgery or pathology, were divided into the treatment group and the control group randomly.In the treatment group 41 cases were treated with intravenous coix seed extract of 100mL or 200mL, 1 time/day,continuously for 2 weeks, a week for a pause, every three weeks as a cycle, a total of 2cycles,combined with chemoradiotherapy,and in the control group 43 cases,were treated with only radiation/ chemotherapy and with an intravenous injection of 5% glucose 500mL plus vitamin C,once a day,continuously for two weeks,and repeatedly after a pause of one week.Both groups were treated with the conventional radiotherapy linear accelerator segmentation radiotherapy. Results:For the treatment group the total effective rate was 60.9%for the control group the rate was 51.2%, and there was no significant difference (P〉0.05);between the treatment group and the control group there was significant difference in the reaction of the gastrointestinal tract, bone marrow suppression, radiation pneumonia, radiation esophagitis correlation analysis (P〈0.05); KPS score changes and changes in body weight in the treatment group compared with the control group were statistically significant;100mL/d compared with 200mL/d dose users was no significant difference (confidence interval contains 0);in the treatment group, radiotherapy plan completion rate of 92.7% was higher than 81.4%, but the difference was not statistically significant; chemotherapy planned completion rate of the treatment group was 90.2%, while 72.1% in the control group and the difference was statistically significant (P〈0.05). Conclusion: Coix seed extract can reduce chemoradiotherapy-induced gastrointestinal reactions,the incidence of bonemarrow suppression,radiation esophagitis and radiation pneumonitis, and improve the quality of life of patients. The severity of adverse reactions is related to coix seed extract doses:the severity of adverse reactions decrease with increasing application of coix seed extract doses.
