Mitral valve repair for mitral insufficiency

摘要 目的回顾性总结36例二尖瓣成形术病人的手术疗效及近中期随访结果。方法 2005年3月~2011年9月,36例非风湿性二尖瓣病变的病人接受二尖瓣成形术,男24例,女12例。年龄23~71岁,平均(48.75±18.03)岁。心功能II级6例(16.7%)心功能III级22例(61.1%)、IV级8例(22.2%)。退行性病变32例,先天性性病变2例,感染性病变1例,缺血性1例。结果无住院死亡,无手术并发症,无SAM现象。术后复查心超32例无或轻微二尖瓣返流,4例轻度二尖瓣返流,二尖瓣返流程度术后明显减轻。术后左房内径(38.25±5.77)mm、左心室收缩末期内径(34.50±7.13)mm、左室舒张末期内径(51.55±6.76)mm,均较术前明显缩小(P〈0.05)。出院时病人心功能均为I级或II级。结论根据二尖瓣病因及病变选用适当的成形技术处理非风湿性二尖瓣关闭不全,可获得良好的临床疗效。 [ Objective] To review the operative results of 36 patients underwent mitral valve repair and the results of short-term and mid-term follow-up. [Methods] The results of mitral valve surgery in patients with non- rheumatic mitral insufficiency who received mitral valve repair (n =36) between March 2005 and September 2011 were analyzed retrospectively. The age of patients ranged from 23 to 71 years (mean 48.75+18.03), 24 patients were males and 12 females. 16.7% (6/36) of the patients were in NYHA class II, 61.1% (22/36) class III, and 22.2%(8/36) class IV. The cause of mitral insufficiency consisted of degenerative disease in 32 cases, congenital lesions in 2 cas- es, infectious lesions in 1 case, and ischemic in 1 case. [ Results ] There was no death and no SAM phenomenon. In all these patients, pre-discharge and follow-up echocardiography showed neither stenosis nor significant regurgita- tion of the mitral valve. Postoperative left atrial diameter (38.25+5.77) mm, left ventricular end systolic diameter (34.50_+7.13) mm, left ventricular end diastolic diameter (51.55+6.76) mm, was significantly reduced compared with preoperative (P 〈0.05). Cardiac function of patients was NYHA class I or II when discharged. [ Conclusion ] Accord- ing to etiology and pathological changes of mitral valve disease ,we select appropriate repair techniques to deal with non rheumatic mitral insufficiency, could get a good clinical effect.
