摘要
目的:评价不同类型人工心脏瓣膜质量及安全性,通过观察瓣膜置换中的超声多普勒血流动力学表全面评价不同类型人工心脏瓣膜的生物相容性和血流动力学表现并与未使用的同类瓣膜进行比较。方法:以计算机检索方法在检索中国期刊全文数据库中(CNKI:1990/2008)检索关于不同类型人工心脏瓣膜置换的临床研究与实验研究的随机对照实验,检索词为"人工心脏瓣膜、血流动力学"。检索后对每项研究的资料结果进行提取、分析。结果:共有13项实验476例人工心脏瓣膜置换患者、6项建立人工心脏瓣膜置换的动物模型的实验研究符合纳入标准,随访结果显示国产机械瓣是一个可靠的心脏瓣膜,与传统机械瓣相比,生物瓣具有更好的血流动力学性能,但其远期在体对血流动力学的影响尚有待进一步随访观察。结论:人工瓣膜包括机械瓣膜和生物瓣膜,作为替代严重受损瓣膜工作的人工瓣膜置换术目前已成为临床改善患者血流动力学异常、提高生活质量的有效治疗方式。但限于实验时间限制,目前还没有更长时间的实验数据,其他有效性指标和安全性,有待更多证据加以评价。
OBJECTIVE: To evaluate the quality and safety of various heart valve prostheses, to determine the biocompatibility and hemodynamics of various heart valve prostheses via ultrasound Doppler hemodynamics during valve replacement, and to compare with the unused valve prostheses.
METHODS: Clinical randomized controlled studies about replacement of various heart valve prostheses were retrieved from CNK with the key words of heart valve prosthesis and hemodynamics from 1990 to 2008. Thereafter, the retrieval data were extracted and analyzed.
RESULTS: Among 13 studies, 476 patients undergoing replacement of heart valve prostheses and 6 animal models based on replacement of heart valve prostheses were included in this study. The following-up results demonstrated that mechanical valve made in China was reliable. As compared with conventional valve, biological valve had well hemodynamics, but the effect on hemodynamics still needs further studies. CONCLUSION: Heart valve prostheses, including mechanical and biological valves, replacing the injured valves are effective treatment methods to improve hemodynamical abnormality and enhance quality of life. However, the efficacy and safety need to be further evaluated.
