摘要
目的:探讨过敏性紫癜(HSP)患儿血清和尿液中黏附分子表达水平及意义。方法:HSP患儿67例,其中无肾损伤37例(NO-HSPN组),有肾损伤30例(HSPN组),正常对照组20例,采用ELISA法检测血清和尿液中可溶性细胞间黏附分子-1(sICAM-1)、可溶性血管细胞黏附分子-1(sVCAM-1)的水平。常规方法检测24h尿蛋白定量。结果:3组血清sICAM-1、sVCAM-1水平差异均无统计学意义(F=2.017和1.408,P均>0.05),尿液水平差异均有统计学意义(F=41.613和14.425,P均<0.05)。NO-HSPN组和HSPN组尿液sICAM-1水平均明显高于正常对照组(P<0.05),但NO-HSPN组和HSPN组之间差异无统计学意义(P>0.05);尿液sVCAM-1水平在HSPN组较NO-HSPN组和正常对照组均明显增高(P<0.05)。HSPN组尿液sVCAM-1水平与24h尿蛋白定量呈正相关(r=0.625,P=0.003)。结论:HSPN患儿尿液sICAM-1和sVCAM-1水平增高,其中尿液sVCAM-1可以用于肾脏免疫炎性损伤的评价。
Aim: To study the serum and urine levels of adhesion molecules of children with Henoch-schonlein purpura ( HSP) . Methods: A total of 37 children with HSP but not HSP nephritis ( NO-HSPN group) ,30 children with HSP ne-phritis ( HSPN group) ,and 20 normal control( normal group) were subjected to measure the levels of soluble intercellular adhesion molecule-1( sICAM-1) and soluble vascular cellular adhesion molecule-1( sVCAM-1) in the serum and urine u-sing ELISA. 24 h urine protein was measured by conventional method. Results: In the three groups,there were significant differences in the urinary levels of sICAM-1 and sVCAM-1( F = 41. 613 and 14. 425,P 0. 05) . The urine levels of sI-CAM-1 in NO-HSPN group and HSPN group were higher than that in normal group( P 0. 05) . Urinary sVCAM-1 in HSPN group was higher than those in NO-HSPN group and normal group( P 0. 05) . In HSPN group,the level of urinary sV-CAM-1 was positively correlated with 24 h urine protein ( r = 0. 625,P = 0. 003) . Conclusion: The levels of urinary sI-CAM-1 and sVCAM-1 are enhanced in children with HSPN,and the level of urinary sVCAM-1 may be used to assess the degree of the immune inflammation of kidney.
