Expression and significance of p21,p185,PCNA in colorectal carcinoma and transitional mucosa adjacent to colorectal carcinoma

摘要 目的 探讨癌基因相关蛋白p2 1,p185及增殖细胞核抗原 (PCNA)在大肠癌癌旁移行粘膜、近端断端大肠粘膜、腺瘤型息肉中表达的意义及其与大肠癌的关系。方法 应用免疫组织化学方法对 40例原发性大肠癌及癌旁移行粘膜 ,2 0例腺瘤型息肉中p2 1,p185及PCNA的表达进行检测。结果 p2 1,p185及PCNA在大肠癌癌旁移行粘膜中的阳性表达率分别为 2 2 5 % (9 40 ) ,17 5 % (6 40 ) ,10 0 % (4 0 40 ) ;p2 1,p185的阳性表达与临床病理分期无关 ;p2 1,p185 ,PCNA表达分级在癌组织及癌旁移行粘膜中存在正相关。结论 在癌旁移行粘膜中p2 1,p185表达异常、细胞增殖力学异常 ,反映癌旁移行粘膜是一种不稳定的癌前状态 。 Objective To study the significance and expresion of rasp21,p125 and PCNA in colorectal carcinoma(CRC), transisional mucosa adjacent to colorectal carcinoma(TMACRC), colonic mucosa of the proximal incision end(CMPIE) and the adenoma polypus(AP). Methods The expression of above protein in CRC, TMACRC, CMPIE in 40 cases, and in AP in 20 cases was investigated by means of streptavidin peroxidase immunohistochemical technique. Results ⑴ Positive expression rate of rasp21, p185, PCNA in TMACRC was 22.5%(9/40), 17.5%(6/40), 100%(40). ⑵ No relationship was found between the expression of rasp21, p185 with cliniopathologic stage in this series. ⑶ Close correlation was found on the expression of rasp21, p185, PCNA between the CRC with TMACRC. Conclusions The expression of rasp21, p185 and the cell proliferative kinetics were abnormal in TMACRC. It indicates that TMACRC is an unstable premalignant change, which might have some correlation with the relapse of CRC.
