摘要
目的:探讨重度肥胖病人行腹腔镜可调节胃束带术(LAGB)前后营养管理方法,观察营养状态变化。方法:重度肥胖LAGB手术病人15例,术前至术后1年进行极低能量饮食干预,并测定术前、术后1、3、6和12个月的体重、超重部分减重%(EWL%)、体重指数(BMI)、骨骼肌、体脂%、血清清蛋白(ALB)、前清蛋白(PA)、总胆固醇(TC)、三酰甘油(TG),以评价能量、蛋白质营养状况及脂代谢变化,随访并发症发生情况。结果:至LAGB术后12个月,EWL%平均达到57.66%,蛋白质营养状况正常,TC、TG水平下降,脂代谢显著改善。15例病人并发症发生情况均有不同程度改善。结论:LAGB围手术期进行极低能量饮食干预及持续的营养管理能改善能量营养过剩,并维持营养状态正常。
Objective: The study aimed to evaluate the effectiveness of laparoscopic adjustable gastric banding(LAGB) surgery in morbidly obese patients.Methods: The nutrition levels of 15 morbidly obese patients were measured pre and post LAGB.Various medical indicators were recorded periodically to assess the energy,protein and lipometabolism.These indicators included preoperative body weight,postoperative body weight at 1,3,6,12 months,percentage of excess weight loss(EWL%),body mass index(BMI),skeletal muscle,body fat percentage,plasma albumin(ALB),prealbumin(PA),total cholesterol(TC) and triglyceride(TG).Complications were also followed up.Results: At 12 months after LAGB surgery,the average EWL% was 57.66%,the protein nutrition sustained normal,the level of TC and TG decreased and lipometabolism was improved significantly.The complications in all patients were improved.Conclusion: A very-low-energy diet intervention and continuous nutrition care can improve over-nutrition and maintain the normal nutrition in the perioperative period of LAGB surgery.
