不同方法留取尿标本在FISH技术诊断膀胱肿瘤的效果评价 被引量:1

Evaluation on effect of different methods of collecting urine specimens in the FISH test for the diagnosis of bladder carcinoma

摘要 目的探讨如何留取有效尿标本,行荧光原位杂交(FISH)技术,以提高膀胱肿瘤的诊出率。方法 2009年4月至2011年4月230例住院患者行FISH试验,其中男性210例,女性20例,年龄7~88岁,平均年龄64.2岁,随机分组留取尿标本,其中一次性留尿100例(A组),分次留尿80例(B组),膀胱冲洗留尿50例(C组)。比较其尿脱落细胞总数超过100个的比例。结果采用一次性留尿脱落细胞超过100个以上占83.0%,采用分次留尿脱落细胞超过100个以上占62.5%,采用膀胱冲洗留尿脱落细胞超过100个以上占84.0%。χ2检验提示,三组差异具有统计学意义(χ2=12.464,P=0.002)。B组与C组有统计学差异(χ2=6.876,P=0.007),A组与B组有统计学差异(χ2=9.876,P=0.002)。A组与C组无统计学差异(χ2=0.024,P=0.538),但膀胱冲洗留尿需要插入尿管及冲洗,为有创伤性方法。结论 FISH试验是膀胱肿瘤的早期辅助诊断手段,采用一次性留取尿标本脱落细胞数优于其他方法,临床上值得推广。 Objective To assess the best method of collecting urine specimens for fluorescence in situ hybridization (FISH) test in bladder carcinoma diagnosis. Methods From April 2009 to April 2011, 230 patients were enrolled in the study. There were 210 males and 20 females, the mean age was 64.2 years (7-88). Urine samples were collected by three different methods: one-time voided urine specimen (100 cas- es, Group A), multi-time voided urine specimen (80 cases, Group B) and bladder irrigated urine specimen (50 cases, Group C). The ratio of exfoliated cells over 100 among the three methods was compared. Results The ratio of exfoliated ceils over 100 in urine specimen of the three methods were 83.3%, 62.5% and 84.0%, the efficiency of the three methods were significantly different (X^2 =12.464, P=0.002). There were significant differences between Group A and B(X^2 =9.876, P=0.002), Group B and C(X^2 =6.876, P=0.007). No difference between Group A and C (X^2 =0.024, P=-0.538). Conclusions FISH is an effective method in early diagnosis of bladder carcinoma. One-time voided urine specimen method should be recommended as a better method to collect urine specimen for FISH test.
