摘要
目的探讨锁定钢板与普通钢板治疗老年肱骨近端骨折疗效。方法选择46例老年肱骨近端骨折患者,按手术方式不同分为肱骨近端锁定钢板组(23例)和普通钢板组(23例)。观察患者术后1、3个月及1年3个时间段骨折愈合、肩关节功能、术后肩关节疼痛以及并发症等情况,采用Neer肩关节评分系统检测,并进行统计学分析。结果手术时间、术中出血情况两组对比差异有统计学意义(P<0.05)。Neer评分:术后1个月,锁定钢板组为42.9分±7.8分,普通钢板组为30.1分±6.3分,两组比较差异有统计学意义(P<0.05);术后3个月,锁定钢板组为72.1分±9.5分,普通钢板组为58.4分±7.1分,两组比较差异有统计学意义(P<0.05);术后1年,锁定钢板组为82.9分±8.6分,普通钢板组为77.0分±7.5分,两组比较差异无统计学意义(P>0.05)。结论与普通钢板比较,锁定钢板治疗肱骨近端骨折手术操作简单,內固定坚强,术中软组织损伤少,对骨质血运的破坏小,更利于骨折愈合,可早期功能锻炼,疗效满意。
Objective To investigate the effect of locking compression plate in elder proximal humerus fracture healing. Methods 46 cases with proximal humeral fractures were selected. All patients was divided into two groups： locking proximal humeral plate （LPHP） group 23 cases and normal plate（NP） group 23 cases. The healing effect, shoulder function, postoperative shoulder pain situation and related complications were observed in 1,3 months and 1 year after surgery. Neer shoulder scoring system for standard, the result used statistical analysis. Results Comparing the operation time and intraoperative bleeding in two groups, the LPHP group were less than NP group, the result was statistically different（P 〈 0.05）. The Neer score in LPHP group was 42. 9±7.8 and NP group was 30. 1±6. 3 in 1 month postoperative, the result was statistically differences（P 〈 0.05 ）. and the Neer score in LPHP group was 72. l±9.5 and NP group was 58.4±7.1 in 3 months postoperative, the result was statistically differences （ P 〈 0. 05 ）. The Neer score in LPHP group was 82. 9±8.6 and NP group was 77.0±7.5 in 1 year postoperatively. There was no statistically differences between two groups of Neer score in 1 year postoperative （ P 〉 0.05 ）. Conclusions Compared with the normal plate, LPHP group in treating proximal humeral fractures have advantages of simple operation, and strong fixation, which reduces the blood of bone damage and peri-operative bleeding, and also can let shoulder function exercise early. It is good for fracture healing.
