摘要
目的观察牵引联合中药湿热敷治疗腰椎间盘突出症(LDH)的临床疗效。方法将224例LDH患者随机分为2组,均予牵引治疗。治疗组112例加用中药湿热敷治疗,对照组112例加用0.9%氯化钠注射液湿热敷治疗,2组均治疗30 d。2组治疗前及治疗15、30 d后分别进行腰腿痛和运动功能评定。结果治疗组治疗15、30 d后和对照组治疗30 d后腰痛问卷(RMQ)评分、视觉模拟(VAS)评分均较本组治疗前减少(P<0.05,P<0.01)。治疗组治疗15、30 d后RMQ、VAS评分较对照组同期减少(P<0.05,P<0.01)。治疗组治疗15、30 d后和对照组治疗30 d后腹肌耐力、背肌耐力均较本组治疗前增强(P<0.05,P<0.01)。治疗组治疗15、30 d后腹肌耐力、背肌耐力较对照组同期增强(P<0.05,P<0.01)。治疗组治疗15、30 d后和对照组治疗30 d后腰椎前屈、腰椎后伸角度均较本组治疗前增大(P<0.05,P<0.01)。治疗组治疗15、30 d后腰椎前屈、腰椎后伸角度较对照组同期增大(P<0.05,P<0.01)。治疗组治疗15、30 d后和对照组治疗30 d后步行能力均较本组治疗前提高(P<0.05,P<0.01)。治疗组治疗30 d后步行能力较对照组同期提高(P<0.05)。结论牵引联合中药湿热敷治疗LDH患者可明显缓解腰腿痛,改善运动功能,疗效肯定。
Objective To observe the clinical therapeutic effect of traction combined with hot-wet compress of traditional Chinese drug on the patients with lumbar disc herniation（LDH）.Methods Totally 224 cases of LDH were divided into treatment group（n=112） and control group（n=112） randomly.Cases in treatment group received traction therapy plus hot-wet compress with traditional Chinese drug,while those in control group received traction therapy plus hot-wet compression with normal saline.All cases received treatment for 30 minutes,once a day.30 days were defined as a course in two groups.lumbocrural pain,abdominal endurance,back muscle endurance,lumbar range of motion and walking ability were performed before treatment and at the end of the 15th and 30th day in all cases.Degree of pain was estimated with Visual analog score（VAS） and Roland-Morrisback pain Questionaire（RMQ）.Results The Score of RMQ and VAS at the 15th and 30th day after treatment in treatment group and at the 30th day after treatment in control group were decreased as compared with those before treatment in two groups（P0.05,P0.01）.The Score of RMQ and VAS at the 15th and 30th day after treatment in treatment group were decreased in comparison with those in control group（P0.05,P0.01）.Abdominal and back muscle endurance,lumbar forward and backward flexion angle in the treatment group were remarkably better than those in the control group at the 15th and 30th day after treatment（P0.05,P0.01）.Walking ability at the 30th day after treatment in treatment group were increased as compared with that in control group（P0.05）.Conclusion The traction combined with hot-wet compress of traditional Chinese drug has definite effect on LDH.
