摘要
目的探讨超声实时监测在经皮肾镜碎石取石术中的应用价值及技巧。方法 248例共276侧肾结石及输尿管上段结石患者,超声实时监测引导建立经皮肾通道,配合钬激光碎石,并在超声引导下寻找残留结石。结果所有病例均穿刺成功并一期碎石取石,一期结石清除率达85.5%(236/276),平均手术时间(78.4±36.5)min(40~198 min),平均出血量(120.5±44.8)ml(80~450 ml)。24例患者术后体温超过38℃,经抗生素药物治疗恢复正常。未发生气胸及严重出血而需要输血的病例。结论术中超声实时动态监测下建立经皮肾通道成功率高,创伤小,安全性高;术中还可协助寻找残留结石,提高结石清除率。
Objective To investigate the clinic value and technics of real-time ultrasound in percutaneous nephrolithotomy.Methods A total of 276 renal calculi or proximal ureteral calculi in 248 cases were studied.The working tracks were all established by real-time ultrasound.The stones were broken by holmium laser.The ultrasound was also performed to find the residual stones.Results The working tracks were successfully established in all patients.The stones were removed in stage one and the clearance rate was 85.5%(236/276).The mean operative time was(78.4±36.5)min(40-198 min),and the mean blood loss was(120.5 ± 44.8) ml(80-450 ml).Twenty-four patients had fever in temperature over 38℃,but could recover after antibiotic therapy.No pneumothorax and serious bleeding happened.Conclusions The real-time ultrasound plays an important role in percutaneous nephrolithotomy as to improve the successful rate in the establishment of the working track.It is also less invasively and more safely in finding the residual stones.
