摘要
目的探讨置换输尿管单J管在部分无管化经皮肾镜碎石取石术后出血患者中的应用效果。方法回顾性分析2014年1月1日至2021年6月1日沈阳市红十字会医院泌尿外科2022例行部分无管化经皮肾镜碎石取石术患者的临床资料,其中术后出血的患者90例,保守治疗无效的患者36例,根据术后出血的止血方法随机分为两组。观察组18例,行经尿道输尿管单J管置换术;对照组18例,行经尿道输尿管输尿管支架管(double J tube,D-J tube)置换术,比较两组患者治疗前后有效率、血细胞比容和血红蛋白等指标。结果观察组有效率高于对照组,差异有统计学意义(P<0.05);两组患者治疗后血红蛋白水平、血细胞比容高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论置换输尿管单J管是一种切实可行的控制经皮肾镜碎石取石术后出血的方法,较置换输尿管D-J管更有效,在行介入动脉栓塞之前可尝试用此方法进行止血。
Objective To investigate the effect of replacement of ureteral single-J tube in patients with bleeding after partially tubeless percutaneous nephrolithotomy.Methods Clinical data for 2022 patients who underwent partial tubeless percutaneous nephrolithotomy between January 1,2014 and June 1,2021 in the Department of Urology,Shenyang Red Cross Hospital were retrospectively analyzed.There were90 patients with 90 patients and 36 patients treated with ineffective conservative treatment.The participants were randomly divided into two groups according to the hemostasis method utilized for postoperative bleeding.There were 18 patients in the observation group underwent a transurethral single J tube replacement,while 18 patients in the control group underwent a transurethral D-J tube replacement.The two groups of patients were compared before and after treatment based on indicators,such as effective rate and hematocrit and hemoglobin levels.Results The effective rate in the observation group was higher than that in the control group(P<0.05).The overall hemoglobin and hematocrit levels in the two groups of patients after treatment were higher than those before treatment.Levels in the observation group were significantly higher than those in the control group(P<0.05).Conclusion Replacement of ureteral single J tube is a feasible method to control bleeding after percutaneous nephrolithotomy.It is more effective than replacement of ureteral D-J tube.This technique can be used to stop bleeding before interventional arterial embolization.
