摘要
目的探讨改良腹腔镜全子宫切除术中盆腔韧带悬吊对盆底重建的影响。方法行全子宫切除术47例作为研究对象,随机分成两组,观察组(A组)25例,行改良腹腔镜全子宫切除术+盆腔韧带悬吊,对照组(B组)22例,行常规腹腔镜全子宫切除术。分析改良腹腔镜全子宫切除术中盆腔韧带悬吊对盆底重建的影响。结果两组手术时间、术中出血量及平均住院日无差异(P〉0.05);观察组术后阴道长度及性生活满意度明显优于对照组(P〈0.05),观察组25例术后无阴道顶端脱垂、张力性尿失禁发生.对照组22例术后一年有ll例阴道顶端脱垂,4例张力性尿失禁,两组相比差异有统计学意义(P〈O.05)。结论改良腹腔镜全子宫切除术+盆腔韧带悬吊对于盆底重建,防止术后盆腔脏器脱垂有积极意义,值得推广.
Objective To investigate the effect of modified Laparoscopic Total Hysterectomy with pelvic Ligaments suspension on pelvic floor reconstruction.Methods 47 women underwent Laparoscopic Total Hysterectomy in our hospital from March 2010 to July 2011.47 cases were recruited in this study.Among them,25 subjects received modified Laparoseopic Total Hysterectomy with pelvic Ligaments suspension(study group), and the other 22 subjects underwent normal Laparoscopic Total Hysterectomy hysterectomy(control group).This is designed to find out treatment effect of modified Laparoseopic Total Hysterectomy with pelvic Ligaments suspension by comparing surgery process,vaginal longitude before and after surgery, vaginal prolapse,stress urinary incontinence and sexual satisfaction by questionnaires between two groups.Results The operating time, bleeding volume and hospital stay did not show significant difference between two groups(P 〉 0.05);there were significant differences between two groups in vaginal longitude and sexual satisfaction.The study group has shown an obvious advantage, over the control group(P 〈 0.05);After surgery, there were no vaginal prolapse and stress urinary incontinence occurred in study group,but 11 patients experienced vaginal prolapses and 4 patients experienced stress urinary incontinence in control group,there were significant differences between them(P 〈 0.05). Conclusion Modified Laparoscopic Total Hysterectomy with pelvic Ligaments suspension should be popularized in pelvic floor reconstruction with its effectiveness in avoiding pelvic organ prolapse.
