摘要
目的:探讨保留子宫动脉上行支筋膜内子宫切除术对患者血清FSH、LH水平的影响及安全性。方法:以笔者所在医院收治的60例子宫良性疾病需行子宫切除术治疗的患者为对象,随机分为观察组和对照组,各30例。观察组行保留子宫动脉上行支筋膜内子宫切除术,对照组行传统腹腔镜筋膜内子宫切除术,比较两组手术相关指标及并发症情况;随访12个月,比较两组血清FSH、LH水平、围绝经期症状和疾病复发情况。结果:两组手术相关指标及并发症发生率比较,差异均无统计学意义(P>0.05);观察组术后6、12个月血清FSH、LH水平均显著低于对照组,差异均有统计学意义(P<0.05);随访12个月,两组均无疾病复发情况;观察组围绝经期症状发生率显著低于对照组,差异有统计学意义(P<0.05)。结论:行保留子宫动脉上行支筋膜内子宫切除术对患者血清FSH、LH水平的影响较小,可显著降低围绝经期症状发生率,改善预后,安全性高,值得推广。
Objective:To investigate the effect of intrafascial hysterectomy with preservation of ascending branch of uterine artery on serum FSH and LH levels and its safety.Method:A total of 60 patients with benign uterine diseases who need hysterectomy in our hospital were randomly divided into the observation group and the control group,30 cases in each group.The observation group received intrafascial hysterectomy with preservation of ascending branch of uterine artery,and the control group received traditional laparoscopic intrafascial hysterectomy.The operation related indexes and complications of the two groups were compared.After 12 months of follow-up,the levels of FSH,LH,perimenopausal symptoms and disease recurrence were compared between the two groups.Result:The operation related indexes and incidence of complications were compared between the two groups,and the differences were not statistically significant(P>0.05).At 6 and 12 months after operation,the levels of serum FSH and LH in the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).After 12 months of follow-up,there was no disease recurrence in both groups,and the incidence of perimenopausal symptoms in the observation group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:The effect of intrafascial hysterectomy with preservation of ascending branch of uterine artery on serum FSH and LH levels of patients is small,which can significantly reduce the incidence of perimenopausal symptoms,improve the prognosis,and is safe and worthy of promotion.
