多重耐药鲍曼不动杆菌外排泵机制的初步研究 被引量:9

摘要 目的研究外排泵在多重耐药鲍曼不动杆菌中的存在情况,为进一步研究其耐药机制奠定基础。方法采用M-H琼脂稀释法,测定添加和不添加抑制剂苯丙氨酸-精氨酸-β-萘胺(PAβN)前后鲍曼不动杆菌对亚胺培南、美罗培南、庆大霉素、环丙沙星和头孢他啶5种药的最低抑菌浓度(MIC)。结果添加PAβN后,74株多重耐药鲍曼不动杆菌对5种药的MIC值下降4倍以上的菌株数分别为亚胺培南45(60.81%)株、美罗培南70(94.59%)株、庆大霉素0(0%)株、环丙沙星4(5.41%)株、头孢他啶45(60.81%)株。结论外排泵机制可能是引起鲍曼不动杆菌对亚胺培南、美罗培南、头孢他啶耐药的主要机制;外排泵机制对环丙沙星和庆大霉素耐药不起主要作用。 Objective To study the phenotype of the efflux pumps in multidrugresistant Acinetobacter baumannii providing foundation for further study the mechanism of multidrug-resistant in Acinetobacter baumannii. Methods The minimum inhibitory concentrations (MIC) of five antibiotics, including imipenem, meropenem, ciprofloxacin gentamicin and ceftazidime with or without efflux pump inhibitor PAf3N were determined by agar dilution method. Results In addition of PAI3N, MIC of 45 strains to imipenem, 70 strains to meropenem, 45 strains to ceftazidime, only 4 strains to ciprofloxacin decreased more than 4 folds, but no one strain to gentamicin. Conclusion Efflux pump mechanism is one of the causes of imipenem , meropenem and ceftazidime resistant in Acinetobacter baumannii,while which is not account for ciprofloxacin and gentamicin.
