摘要
目的探讨MRI对脑静脉畸形(CVM)的诊断价值。方法收集我院9例CVM患者资料进行回顾性分析。3例行CT平扫检查,9例行MRIT1WI、T2WI、T2FLAIR、T1WI增强扫描,其中有3例行3DTOFMRA扫描,2例行VENBOLDSENSE扫描。结果 CVM位于幕下5例,幕上4例。3例CT平扫均未见CVM直接征象,其中2例发现异常病灶,1例未见异常。5例MRI平扫显示髓静脉呈长T1长T2信号,中央静脉干呈长T1短T2信号或长T1长T2信号,FLAIR上髓静脉为稍高信号,中央静脉干为稍低信号或稍高信号,增强呈典型的"水母"征或"树根"征表现。2例MRI平扫显示条管状流空低信号,增强呈条状高信号。1例MRI平扫显示类圆形等长T1、等长T2信号,中心可见流空信号。增强MRA显示髓静脉呈放射状汇入中央静脉并见引流入上矢状窦。1例术前只诊断为血管畸形,未能明确分型。结论 MRI多序列成像对脑静脉畸形绝大多数可明确诊断,MRI增强及VENBOLDSENSE序列是发现脑静脉畸形最敏感的方法,应做为CVM的首选检查方法。
Objective To investigate the value of MRI in diagnosis of cerebral venous malformation (CVM). Methods 9 cases in our hospital patients with CVM data were analyzed retrospectively. 3 routine CT scan examination, 9 routine MRI T1WI, T2WI, T2FLAIR, T1WI enhanced scan, of which three routine 3D TOF MRA scans, two routine VEN BOLD SENSE scan. Results 5 cases were the infratentorial, 4 cases of supratentorial. 3 cases CT scan showed no direct signs of CVM, including two cases of unusual lesions, 1 case was normal. MRI scan showed five cases of medullar veins elongated T1 long T2 signal, the central vein stem elongated T1 and short T2 signal or long T1 long T2 signal, FLAIR signal slightly higher on the medullar vein, the central vein was slightly lower or slightly higher signal , enhancing showed a typical "Jellyfish" sign or the "root" sign performance. MRI scan showed two cases of tubular flow void of low signal to enhance strip showed high signal intensity. 1 case MRI scan shows round-like and long T1, such as long-T2 signal, the center shows the signal flow void. Enhanced MRA showed medullar veins radial import into the central vein and superior sagittal sinus, see Citation. Only 1 case could not be divided into specific sub-type preoperatively. Conclusion Multi-sequence MRI imaging of the vast majority of cerebral venous malformations can be diagnosed, MRI enhancement and VEN BOLD SENSE sequence is the most sensitive method in CVM, and should be preferred as the CVM inspection methods.
