精子质量和功能参数与IVF受精率和优胚率的关系探讨 被引量:12

Exploration on the relationship between quality and function parameters of sperm and fertilization rate,good quality embryo rate in in vitro fertilization

摘要 目的:对优化处理后前向运动精子总数、精子正常形态百分率和精子顶体反应阳性率,这三项能体现精子质量和功能的参数与IVF受精率和优胚率之间的关系进行探讨。方法:分析226对接受IVF治疗的不育夫妇优化处理后前向运动精子总数、精子正常形态百分率、精子顶体反应阳性率、IVF受精率和优胚率等数据,观察精子质量和功能参数对IVF受精率和优胚率的影响。结果:当处理后前向运动精子总数<5×106时,IVF受精率极显著低于其他组(P<0.01);当处理后前向运动精子总数≥5×106时,IVF受精率不随处理后前向运动精子总数的增加而上升(P>0.05);处理后前向运动精子总数对优胚率无显著影响(P>0.05);精子正常形态百分率和顶体反应阳性率对IVF受精率和优胚率均无显著影响(P>0.05)。结论:优化处理后前向运动精子总数与IVF受精率相关,与优胚率无关;正常精子形态和顶体反应与IVF受精率和优胚率无关。 Objective:To explore the relationship between quality and function parameters of sperm and fertilization rate of in vitro fertilization(IVF),good quality embryo rate according to the amount of progressive sperm after optimal treatment,the proportion of sperm with normal morphology and positive rate of acrosome reaction.Methods:The data including the amount of progressive sperm after optimal treatment,the proportion of sperm with normal morphology,positive rate of acrosome reaction,fertilization rate of IVF and good quality embryo rate of 226 couples receiving IVF treatment were analyzed;the effects of quality and function parameters of sperm on fertilization rate of IVF and good quality embryo rate were observed.Results:When the amount of progressive sperm after optimal treatment was less than 5×10^6,the fertilization rate of IVF was lower than those in other groups(P〈0.01);when the amount of progressive sperm after optimal treatment was more than 5×10^6(including 5×10^6),the fertilization rate of IVF did not increase with the increase of amount of progressive sperm after optimal treatment(P〉0.05);the amount of progressive sperm after optimal treatment had no apparent effect on good quality embryo rate(P〉0.05);the proportion of sperm with normal morphology and positive rate of acrosome reaction had no apparent effect on fertilization rate of IVF and good quality embryo rate(P〉0.05).Conclusion:The amount of progressive sperm after optimal treatment is related to fertilization rate of IVF,which is not related to good quality embryo rate;normal morphology of sperm and acrosome reaction are not related to fertilization rate of IVF and good quality embryo rate
