精子形态、顶体反应与卵子受精率的相关性研究 被引量:22

摘要 目的 研究体外受精(in vitro fertilization,lVF)中精子形态、顶体反应与卵子受精率之间的关系,分析精子 各参数在IVF受精率方面的参考意义。方法 分析男方精液标本125份,进行常规精液分析、精子形态及精子顶 体反应分析。观察受精卵1 475个,记录受精和未受精卵的数目,并分析精子各参数与卵子受精率之间的相关性。 结果 根据精子形态分析将精子正常形态分为4组,<14%、14%~24%、25%~34%和≥35%,其相对应的卵子受 精率分别为61.5%、66.7%、74.8%和79.1%,各组间相比差异有显著性,精子形态与卵子受精率有明显的相关性。 精子的顶体反应率分为3组,分别为≤3%、4%~7%和≥8%,其对应的卵子受精率分别为75.2%、75.O%和 74.5%,各组间相比差异无显著性,在体外精子顶体反应率与IVF受精率无明显的相关性。结论 在IVF中,精子 的形态对卵子的受精率有着明显的影响,卵子对正常形态的精子有一定的选择性。在体外,精子顶体反应率的高低 与卵子受精率没有明显的相关性。因此,精子形态分析和顶体反应二项参数,在自然周期和IVF周期中,对受精功 能的评价有着不同的参考价值和意义;同时,在人类辅助生殖技术实施过程中可以作为选择不同辅助生殖技术的依 据。 Objective To investigate the relationship between ovum fertilization rate and sperm morphology, acrosome reaction in IVF and to explore the influence of parameters of sperm on fertilization rate. Methods 125 semen samples were analyzed with routine analysis, sperm morphology and acrosome reaction and 1475 ova either fertilized or unfertilized were observed in order to explore the relationship between the sperm parameters and ovum fertilization rate. Results Four groups were divided according to the percentage of normal sperm morphology: 〈 14 %, 14 % - 24 % ,25 % - 34 % and ≥35 %, the ovum fertilization rate was 61.5 %, 66.7 %, 74.8 % and 79.1% corresponding. There was significant difference among these four groups. The result showed that there was positive correlation between sperm morphology and ovum fertilization rate. Then, three groups were divided according to sperm acrosome reaction rate: ≤3 % ,4 % - 7 % and≥ 8%, the ovum fertilization rates were 75.2 %, 75.0 % and 74.5 % corresponding. There was no difference in three groups. The result showed that there was no correlation between sperm acrosome reaction and ovum fertilization rate in vitro. Conclusion In IVF , the sperm morphology may have significant influence on ovum fertilization rate, and human ova are selective for sperms with normal morphology. In vitro, the acrosome reaction of sperm does not influence the ovum fertilization rate. The morphology and acrosome reaction of sperm may be two important parameters to evaluate the function of fertilization.
