摘要
目的探索减少医院患者退药现象的有效模式和方法。方法对我院2009年度门(急)诊药房、住院药房308份退药申请单进行分类统计。结果主要退药原因有医嘱变更、药品不良反应、处方错误、调剂差错等。结论提供药学咨询服务、开展患者用药教育、加强医患的有效沟通、提高服务水平是减少退药现象的有效方法。
Objective To explore the measures to reduce drug repercussion cases.Methods The 308 drug percussion applications in(emergency) pharmacy and hospital pharmacy from January 1 to December 31 in 2009 were classification and statistical analysis.Results Of the total 308 cases,main causes were changes of doctor's order,adverse drug reaction,prescription errors and dispensing mistakes.Conclusion The effective way to reduce drug repercussion cases were to improve clinical pharmaceutical care and patient education,to establish good rules and to raises the service level.
