摘要
目的探讨手指末端指腹缺损修复的最佳治疗方案。方法依据伤情将２０３例手指末端指腹缺损分为２型、４个亚型，并依据手指末端指腹缺损的分型分别进行植皮，用Ｖ－Ｙ皮瓣、鱼际皮瓣、邻指皮瓣、指掌侧推进皮瓣、带血管神经蒂岛状皮瓣、游离皮瓣等７种不同方法修复。结果术后手指末端指腹缺损修复的外形及功能满意。结论对Ⅰａ、Ⅱａ型缺损以植皮为主，最好采用全厚皮片；对Ⅰｂ、Ⅱｂ型缺损以带血管神经蒂岛状皮瓣和游离皮瓣修复为主，并强调优先恢复拇。
Objective To study the best effect of reconstruction of skin defect in distal bellyfinger. Method According to the different injuries of skin defect in distal belly-finger,203 cases were divided into two types and four groups.Based on the types of skin-defect in distal belly-finger,skin graft,V-Y flap,thenar flap,adjacent flap,wandering flap,vascular nerve pedicle flap were taken in reconstruction.Result The external appearance and function of the reconstruction of skin defect in distal belly-finger was satisfactory.Conclusion The reconstruction of type Ⅰa and Ⅱa should be put skin graft first and the best for skin graft.The reconstruction of type Ⅰb and Ⅱb should be put graft of vascular nerve pedicle flap and wandering flap first and be emphasized the sensity on thumb,index and middle finger.
