摘要
目的探讨大肠癌患者外周血中PGE2、IL-2、T淋巴细胞亚群在手术前后的动态变化和相互关系及临床意义。方法 61例大肠癌患者按有无转移分成两组:无远处转移组46例,远处转移组15例;其中根治性切除41例(根治组),姑息性切除11例(姑息组),剖腹探查9例(剖腹组)。对照组为健康体检者。检测患者手术前后外周血PGE2、IL-2、CD4+、CD8+含量。结果大肠癌患者术前外周血PGE2水平明显升高,IL-2水平和CD4+/CD8+值降低,IL-2和PGE2呈显著负相关。手术切除原发肿瘤后外周血PGE2显著下降,IL-2水平和CD4+/CD8+值显著升高;其中根治组和对照组相比差异无统计学意义,姑息组和剖腹组术后外周血PGE2水平仍显著高于对照组。结论 PGE2参与了大肠癌患者术前存在的免疫抑制过程。非甾体抗炎药与外源性IL-2联合应用以预防和治疗大肠癌理论上具有可行性。
Objective To determine the dynamic change of the levels of peripheral blood PGE2,IL-2 and T-lymphocyte subsets, and explore the relationship between PGE2 and IL-2 in patients with colorectal carcinoma pre-and post-operation.Methods 61 patients with colorectal carcinoma were accepted in the study, 15 case had simultaneously distant metastasis and 46 patients did not show any obvious distant metastasis. 11patients underwent palliative resection and 9 patients laparotomy. 41 patients received resection. 30 people found healthy at physical examination were considered as controls. The levels of peripheral blood PGE2,IL-2,CD^4＋ and CD8^＋ and pre-and post-operation in 61 patients with colorectal carcinoma were assayed respectively.Results The patients with colorectal cancer had a high concentration of peripheral blood PGE2 before operation. The levels were considerably higher than the normal in this study. But the level of IL-2 and CD^4＋/CD8^＋ in peripheral blood were lower than the control group before surgical operation. The peripheral blood PGE2 had a negative correlation with IL-2 and CD^4＋/CD8^＋. The levels of PGE2 were lower after the original tumor resected and the levels of IL-2 and CD^4＋/CD8^＋ were higher after operation than before operation, The level of PGE2 and decreased almost to normal in the patients with radical resection, but were still higher in those with palliative resection.Conclusion PGE2 participates in the immunologic hypofunction existing in the patients with colorectal carcinoma before operation. It is feasible in theory that NSAID and exogeous IL-2 are applied to prevent the metastasis or recurrence of colorectal carcinoma.
