摘要
目的探讨计算机辅助手术导航系统(CASNS)指引下经皮行球囊扩张椎体成形术治疗老年骨质疏松性椎体压缩骨折的临床应用及治疗效果。方法对49例老年骨质疏松性椎体压缩骨折患者的73个椎体,在计算机辅助手术导航系统指引下经皮自椎弓根向椎体内穿刺,行球囊扩张椎体,纠正脊柱后凸畸形,并注入聚甲基丙烯酸甲酯(PMMA),测量计算手术前/后椎体前/后缘高度比值。治疗效果采用Husk isson止痛视觉评分法及椎体形态、基本活动能力变化综合评价。结果45例患者2 d内缓解疼痛,21例患者停用止痛药,4例患者疼痛缓解轻微。手术前后椎体前/后缘高度比值恢复80%,差异有统计学意义(P<0.05)。无并发症发生。结论CASNS指引下经皮行球囊扩张椎体成形术治疗老年骨质疏松性椎体压缩骨折是一种精确、快捷、安全、有效的微创疗法,随着技术的日益成熟,将广泛应用于临床。
Objective To investigate the clinical application and efficiency of percutaneous balloon dilatation vertebroplasty in the treatment of osteoporotic vertebral bady compression fracture under the guidance of the computer-aided surgical navigation system(CASNS).Methods 49 cases of osteoporotic vertebral compression fracture with 73 vertebral body compressed,under the guidance of the computer-aided surgical navigation system,were teated by using percutaneous puncture pass through the vertebral arch root to the vertebral body,balloon dilatation to correct kyphosis deformity,and to inject polymethylmethacrylate(PMMA),measuring the vertebral body's height before and after the operation,then accounting the ratio of them.Using Huskisson acesodyne visual score method,the vertebral body shape and the basic activities were detected to evaluate the therapeutic effect.Results The pain was relieved in 45 patients within 2 days,21 patients stopped after using anodyne,and the pain was slightly relieved in 4 patients.Before and after surgery the restoration of height ratio was 80%(P 0.05),which had the significant difference.No complications were found.Conclusion The percutaneous balloon dilatation vertebroplasty in the treatment of osteoporotic vertebral compression fracture under the guidance of the computer-aided surgical navigation system is a precise,fast,safe,effective and minimally invasive therapy,with the technique becoming more sophisticated,which will be widely used in clinic.
