CTA对颅内微小动脉瘤的诊断价值并与DSA对照研究 被引量:6

摘要 目的:探讨16层螺旋CT血管成像和数字减影造影在颅内微小动脉瘤(VSD)诊断及治疗中的应用价值。方法:对843例原发性蛛网膜下腔出血的动脉瘤疑似患者行CTA和DSA检查。CTA后处理采用多平面重建(MPR),最大密度投影(MIP),容积再现(VR)。DSA常规摄正、侧位、双侧斜位片。结果:经手术及介入栓塞治疗证实39枚微小动脉瘤,CTA诊断36枚,DSA诊断35枚。减影后CTA与DSA比较,对瘤体直径≤3mm的VSA的判断,无明显统计学差异,敏感性为92.3%,特异性为85.7%,准确性为91.3%。结论:CTA诊断微小动脉瘤是一种可靠方法,在临床诊断上与DSA基本等同,在对临床治疗提供帮助上,CTA明显优于DSA。 Objective:To evaluate the value of 16-slice spiral CT angiography(CTA) and digital subtraction angiography(DSA) in the diagnosis of very small aneurysms.Methods:CTA and DSA were performed in 843 patients who were highly suspected with intracranial aneurysms.VR,MIP,MPR were used as a post-processing technique of CTA.DSA usually shoot the slice of head AP side and an incline.Results:39 cases had been confirmed by operations and interventional procedures.36 VSA cases were detected by CTA,and 35 were detected by DSA.There were no statistical differences between CTA and DSA(P≤3 mm) after subtraction process,with the rate of sensitivity,specificity and accuracy were 92.3%,85.7% and 91.3% respectively.Conclusion:CTA is a reliable and efficient technique in diagnosing the VSA.The results of CTA and DSA are almost the same in clinical diagnosis.As for providing the help for clinical treatment,CTA is obviously superior to DSA.
