64层螺旋CT同步减影技术在颅内动脉瘤中的应用价值 被引量:5

Applied value of intracranial aneurysms using 64-slice spiral computed tomograghy synchronous subtraction angiography

摘要 目的:探讨64层螺旋CT同步减影技术(SSCTA)在颅内动脉瘤诊断中的应用价值。方法:回顾性分析31例经手术和(或)DSA确诊为动脉瘤患者CTA资料,所有患者均行同步减影CTA检查,用容积再现(VR)和最大密度投影(MIP)重组方式显示;以手术和(或)DSA为标准对图像进行分析。结果:SSCTA可以完全去除颅底复杂的骨结构干扰,得到较高质量的CTA图像;可以大幅度降低CTA的三维重组时间;可以充分显示颅内动脉瘤部位、大小、形态,局部特写有利于显示动脉瘤体,瘤颈与载瘤动脉的关系。SSCTA共检出动脉瘤34个,3例多发;其中颈内动脉颅底段21个,大脑中动脉3个,前交通动脉瘤7个,椎动脉瘤3个;最大者15.7mm×15.3mm,最小2.3mm×2.2mm;经与手术及DSA结果比较,SSCTA检出动脉瘤的敏感性100%,特异性100%。结论:SSCTA对颅内动脉瘤的诊断具备安全、无创、快捷,且血管图像分辨率高,对靠近颈内动脉颅底部的动脉瘤显示具有较高的特异性及敏感性,可作为临床首选的颅内动脉瘤筛选方法。 Objective:To study the value of application of 64-slice Spiral Computed Tomograghy Synchronous Subtraction angiography(SSCTA) in the diagnosis of intracranial aneurysm.Methods:SSCTA was performed in 31 patients who were suspected of having intracranial aneurysm(MIP).The diseases were displayed using volume rending technology(VR) and maximum intensity projection,then the manifestations were compared with DSA or surgical findings.Results:SSCTA could completely eliminate skull and reduce the reconstruction time significantly.SSCTA could clearly display spacious anatomic of aneurysm,including its origin,size,neck,and the relationship between the aneurysms and surrounding structures were accurately displayed.By SSCTA,34 intracranial aneurysms were detected in 31 cases,2 were seen in 3 cases,the sensitivity and specificity were both 100%.Conclusion:SSCTA is a safe,non-invasive and rapid technique,with improved contrast of vessel in image quality.It has high specificity and sensitivity in the diagnosis of aneurysms in cranial base.SSCTA may be the first choice for diagnosis intracranial aneurysms.
