摘要
目的研究颌面部撞击伤导致的下牙槽神经血管束间接损伤的特点。方法18只实验犬随机均分为6组,以重量0.26kg的铁质帽状圆柱体撞击致伤犬右下颌骨体部,但不直接伤及下牙槽神经血管束;分别于伤后6h、24h、3d、7d、2周及4周处死实验犬,对下牙槽神经、动脉的间接损伤进行组织病理学研究和超微结构观察。结果伤区附近的下牙槽神经和下牙槽动脉均受到一定程度的间接损伤。损伤程度不甚严重,随着与伤区距离的增加而递减;损伤是暂时的、可逆的。结论实验结果提示,在救治颌面部撞击伤伤员,尤其是整复下颌骨骨折时,除非下牙槽神经血管束发生断裂需结扎,否则应予以妥善保护,避免在救治过程中损伤。
Objective This study was designed to determine the characteristics of indirect injury of inferior alveolar neurovascular bundle caused by impact injury of mandible in dogs. Methods Eighteen dogs were divided equally into six groups with random allocation. The right mandible of each dog was stroken by cylindroid irony cap weighted 0.26kg, but inferior alveolar neurovascular bundle was not injured directly. Then, the animals were killed 6h, 24h, 3d, 7d, 2wk and 4wk later, respectively, and the histopathologic and ultrastructural changes of inferior alveolar nerve and inferior alveolar artery were investigated. Results The inferior alveolar nerve and inferior alveolar artery close to the impact region were injured with some degree; The effect of the trauma decreased progressively as the distance to the impact site increased; the injuries were temporary and reversible. Conclusion When treating a patient of maxillofacial impact injury, the inferior alveolar neurovascular bundle should be well protected to avoid injury during treating.
