发育性偏颌畸形患者的下颌骨三维形态分析 被引量:16

摘要 目的:利用螺旋CT三维重建测量分析发育性下颌偏斜畸形患者下颌骨硬组织的特征性变化。方法:选择成人下颌偏斜畸形患者20人,比较三维重建后下颌骨偏斜侧与对侧在矢状向、侧向及垂直向上的差异。结果:下颌骨双侧在三个方向上的线距测量和角度测量均出现明显的不对称,其中升支长度、升支高度、喙突至下颌平面高度、下颌角点成角、髁突喙突成角、髁突内外径长轴成角及升支侧方倾斜角,双侧比较差异有统计学意义(P<0.05);总长度、喙突高度、髁突高度、髁突顶点成角、升支矢状向倾斜角及相对下颌平面成角,双侧比较差异均有统计学意义(P<0.01)。结论:发育性偏颌畸形患者的下颌骨出现立体结构的扭转变形,在进行正畸-正颌联合治疗时应予以注意. AIM: To evaluate the asymmetry of the mandible in adult patients with mandibular developmental asymmetry by using CT scan and three-dimensional reconstruction model. METHODS : After the reconstruction of the mandible, the mandibular developmental asymmetry was assessed by measuring the difference between both shifted side and nonshifted side of 20 subjects in coronary, sagittal and axial dimensions respectively. RESULTS: Analysis with a paired t test of the mean difference between the shifted side and the nonshifted side showed that patients suffering mandibular developmental asymmetry displayed significant asymmetries in all three dimensions with both linear measurements and angular measurements. There were significant differences between the shifted side and the nonshifted side in RL, R height, Scp-MP, Sco-Go-Me(SGM) , Scp-Go-Sco(SGS) , CLA and R-FH(P〈0.05) ; and in FL, Scp-FH, Go-FH, InfSigFH, Sc height, Sco'-Sco-Go (SSG), R-MS and R-MP (P 〈0.01 ). CONCLUSION: The asymmetries of the mandible are reflected both in linear measurements and angular measurements, which leads to the transfiguration of the mandible and deserves great attention during the orthodontic and orthognathic treatment.
