摘要
目的研究尿毒症患者胸部影像征象及临床表现的特征。方法与正常对照组比较,分析180例尿毒症患者胸部影像平片的表现及临床表现。结果异常的胸部影像征象包括:(1)肺纹理改变120例;(2)肺瘀血140例,肺间质水肿105例;(3)肺泡性肺水肿69例;(4)胸腔积液63例,胸膜增厚47例;(5)心影增大72例;(6)肾性骨病23例。相关的临床表现包括:140例胸闷、气促、咳嗽、咳痰,其中32例血痰,呼吸困难86例,肺部闻及湿罗音108例,体温正常并排除肺炎;心界扩大并排除心脏器质性病变72例。180例均测量双上肺静脉血管蒂宽径(VPW),其值大于正常对照组(P<0.01)。结论正确分析尿毒症患者胸部影像平片征象及临床表现,将之与其它原因引起的胸部影像表现鉴别,有助于对此病制订合理的治疗方案.
Objective To study the imaging features on plain chest radiography film of cases with hemodialysis due to uraemia and clinical appearances of the patients. Methods In comparison with control group, clinical and radiographic features of 180 cases with ureamia with hemodialysis had been analyzed. Results Abnormal radiographic signs were as follows： （1） lung marking changes in 120 cases; （2） mesenchymal pulmornary edema in 105 cases; （3） alveolar pulmonary edema in 69 cases; （4） pleural effusion in 63 cases, pleura thickened in 47; （5） enlarged heart shadow in72 cases; （6） osseous lession secondary to uraemia in23 cases. The clinical symptoms of these patients include breath hard and cough in 140 cases, emptysis in 32 cases, and dyspnea were in 86 cases. The rale occurred in 108 cases with normal body temperature, from whom pneumonia was excluded. Enlargement of heart was found in 72 cases without essential cardiac lesion. Bilateral VPW in 180cases was larger than that of control group （P〈0.01 =. Conclusion It is very important to analyze correctly the image signs on chest radiography of patients with ureamia with hemodialysis and differentiate them from abnormal chest radiography signs caused by other chest diseases for determining the therapy scheme in patients with ureamia.
