The effect of hyperbaric oxygen therapy on the levels of serum sICAM-1 and sVCAM-1 in patients with acute cerebral infarction

摘要 目的探讨高压氧(HBO)治疗对急性脑梗死患者血清可溶性细胞间粘附分子-1(sICAM-1)、可溶性血管细胞粘附分子-1(sVCAM-1)含量的影响及临床意义。方法采用酶联免疫吸附法对20名健康人(对照组)、50例HBO加常规治疗组(HBO组)和50例常规治疗组(常规组)的急性脑梗死患者在入院1、7、14d进行血清sICAM-1、sVCAM-1含量的动态监测,并于入院1、14d对神经功能缺失进行评分,评价2组患者疗效差异。结果入院1d2组脑梗死患者血清sICAM-1、sVCAM-1含量明显高于对照组(P〈0.01);入院7、14d,同组内患者血清sICAM-1、sVCAM-1含量逐渐降低,2个时间点相比差异有统计学意义(P〈0.01);入院1dHBO组与常规组比较,两指标差异无统计学意义,而7、14d,两指标均低于对应时间点的常规组(P〈0.01)。入院14dHBO组神经功能缺失评分明显低于常规组,2组总有效率差异有统计学意义(P〈0.05)。结论HBO治疗能降低脑梗死患者血清sICAM-1和sVCAM-1的含量,有效抑制粘附分子的表达,提高临床疗效。 Objective To characterize the effect of hyperbaric oxygen(HBO) therapy on the levels of serum soluble intercellular adhesion molecule-1 ( sICAM-1 ) and soluble vascular cell adhesion molecule-1 (sVCAM-1) in patients with acute cerebral infarction (ACI) and their clinical significance. Methods The levels of sICAM-1 and sVCAM-1 in serum were measured by ELISA on clay 1, 7 and 14 after hospitalization in 50 ACI patients treated with HBO and general therapy ( HBO group ) , 50 treated with general therapy only (general group) and 20 healthy people( control group). The clinical neurological deficit scores in ACI patients were evaluated on day 1 and 14. And the therapeutic effect in HBO group were compared with that in genenal group. Results The levels of sICAM-1 and sVCAM-1 in serum were significantly higher in ACI patients of both groups than those in controls on clay 1 after hospitalization (P 〈0.01 ). The decrease of two indicators of ACI patients in each group on clay 7 and 14 were more obvious ( P 〈 0.01 ). There were no differences in the indicators between both of HBO group and general group on clay 1, but on clay 7 and 14, the indicators in HBO group were decreased more obviously than those in general group at each corresponding time (P 〈 0.01 ). On clay 14 after hospitalization, the neurological function deficiency scores of HBO group were much less than that of general group. Total effective rate in HBO group was obviously higher than that in general group ( P 〈 0.05 ). Conclusions HBO therapy could decrease the levels of sICAM-1 and sVCAM-1 in serum in ACI patients, inhibit the expression of adhesion molecules and increase the clinical efficiency.
