5444例原发性恶性骨肿瘤组织病理学统计分析 被引量:19

摘要 目的:从大宗病例分析总结原发性恶性骨肿瘤的发病情况及构成比率。方法:通过对组织病理学确诊为原发性恶性骨肿瘤5444例的临床病理统计分析,观察各种原发性恶性骨肿瘤的构成比及其在不同年龄组、性别及部位的发病情况。结果:5444例原发性恶性骨肿瘤中男女比例为1.6:1,年龄组中11~31岁占55.70%。病变位于下肢长管骨占53.09%。其中主要的肿瘤发病数从高到低依次为:骨肉瘤(43.66%)、软骨肉瘤(20.52%)、纤维肉瘤(6.02%)、恶性淋巴瘤(5.62%)、尤文氏肉瘤(5.40%)、脊索瘤(5.36%)、骨髓瘤(5.03%)、恶性纤维组织细胞瘤(3.97%)、恶性骨巨细胞瘤(2.50%)。结论:原发性恶性骨肿瘤较多发生于男性,好发年龄为11~30岁。病变部位较多位于下肢长管骨,其中以骨肉瘤最多见。原发性恶性骨肿瘤在我国的发生数、构成比等与北美等地区有着某些差异。 Objective: To investigate the constituent ratio and incidence of primary malignant bone tumors in a big amount of cases. Methods: Pathological survey and analysis of 5444 cases with primary malignant bone tumors were conducted. The basic data supplied in this study was mainly obtained during the period from 1960-2000. Results: Among the 5444 cases, the male:female ratio was 1.6:1. Patients between 11 and 31 years old amounted to 55.7% of the total cases, and patients with a diseased region in the long bone of the lower limb totaled 53.09%. The constitutional ratio of the cases, in order from highest prevalence to lowest prevalence, is as follows: osteosarcoma (43.66%), chondrosarcoma (20.52%), fibrosarcoma (6.02%), malignant lymphoma (5.62%), Ewing's sarcoma (5.40%), chordoma (5.36%), myeloma (5.03%), malignant fibrohistiocytoma (3.97%) and malignant giant cell tumor of bone (2.5%). Conclusion: Malignant primary bone tumors were more common in male patients, with a predilection for diagnosis between ages 11 and 30. The long bone of lower limbs was the most common site of the disease. Among the 5444 cases studied, osteosarcoma was the most prevalent subtype of malignant primary bone tumor. There were some differences in the number of cases and the constituent ratio when comparing data from China and North America.
