摘要
结合临床对慢性萎缩性胃炎(CAG)伴胃癌前病变(PLGC)的研究观察,笔者将中医的整体学说之脏腑理论与CAG及PLGC及其涉及的相关人体免疫学、细胞动力学及血管生成学说等作一密切联系。本文就脾胃虚损说在慢性萎缩性胃炎癌前病变发病机制中的重要意义,分别从脾胃虚损,中气不足;湿热壅滞,升降失职;气阴两虚,纳运失调三方面加以论述。其中脾胃功能的失调,在本病的发病机制乃为首要。脾胃虚损说将丰富CAG及PLGC的病机学内涵,更好的指导临床。
Combining with the research of CAG and PLGC, we suggest that it closely related to immune functions, variation of cell dynamics, angiogenesis and so on. We are of the opinion that combining the above fields with the concept of organic whole and the theory of ZangXiang is the developing way of the TCM theory. The paper discusses the three aspects which are insufficiency of the spleen-Qi and the stornach-Qi, disfunction of ascending and descending of the spleen clue to dampness and heat, disfuncfion of transportation and transformation because of asthenia of both Qi and Yin. Disfunction of the spleen and the stomach is the most important part of the pathogenesis. The theory of asthenia of the spleen and the stomach will enrich the pathogenesis and will guide the clinical diagnosis and therapy.
