摘要
目的探讨髋臼骨折的螺旋CT扫描分型。方法回顾性分析131例髋臼骨折患者的螺旋CT扫描资料,以髋臼壁或柱骨折的位置及骨折延伸的范围为分类标准,不包括股骨头脱位及骨折的碎裂程度。结果131例髋臼骨折的CT表现分为3类9个亚型。Ⅰ类:单柱或/和壁骨折,其中ⅠA前壁或/和后壁骨折27例,ⅠB单柱骨折26例,ⅠC单柱骨折累及髂骨翼或坐骨或/和耻骨17例,ⅠD单柱骨折同时累及髂骨翼及坐骨或/和耻骨3例;Ⅱ类:其中ⅡA局限于髋臼的双柱骨折16例,ⅡB双柱骨折累及髂骨翼14例,ⅡC:双柱骨折累及坐骨或坐骨耻骨联合处或耻骨支5例,ⅡD:双柱骨折同时累及髂骨翼及坐骨、坐骨耻骨联合处或耻骨支11例;Ⅲ类:双柱骨折完全分离,即浮动髋臼12例。结论以CT扫描为基础的髋臼骨折分类,包括所有髋臼骨折的类型,对髋臼骨折的诊断、治疗有重要指导意义。
Objective To discuss the classification of acetabular fractures on spiral CT scans. Methods Spiral CT imaging features of 131 cases of acetabular fractures were retrospectively analysed. The fracture patterns were classified according to the position of the acetabular column or wall and the extension beyond the acetabulum. Fracture and dislocation of the femoral head were excluded from this study. Results In this series, the CT manifestations could be classified into 3 types ( including 9 subtypes). Type Ⅰ included single-column or/and wall fracture, in which subtype Ⅰ A fracture involved in the anterior or posterior wall(n = 27) , subtype Ⅰ s was single anterior or posterior column fracture ( n = 26) , subtype i c was single-column fractures involving the iliac ala or ischium or/and pubis ( n = 17 ), subtype Ⅰ D was slngle-column fractures involving the iliac ala and ischium or/and pubis ( n = 3 ). Type Ⅱ fracture included both the anterior and posterior columns of the acetabulum, in which subtype Ⅱ A fracture was limited to the acetabulum( n = 16), ⅡB fracture involved in iliac wing( n = 14), Ⅱ c involved in ischium and pubic ramus( n =5 ) , and Ⅱ D involved in iliac wing and ischium or pubic ramus beyond the acetabulum( n = 11 ). Type Ⅲ fracture was defined when dual-column fracture was completely separated from the axial skeleton both anteriorly and posteriorly, ie, floating acetabulum ( n = 12 ). Conclusion Spiral CT scans provides a basis for classification of acetabular fractures. This classification comprises all patterns of acetabular fractures on which to guide the diagnosis and treatment of acetabular fracture.
