摘要
目的 探讨心率及心率波动对16层螺旋CT冠状动脉成像(16-MSCTA)图像质量的影响.方法 不同心率、心率波动患者89例行16-MSCTA,以0.4秒/周扫描.根据心率分为下列四组:A组,<60次/分(beat per minute, BPM);B组,60~69 BPM;C组,70~79 BPM;D组,≥80 BPM.根据心率波动范围上述患者分为三组:第一组,范围≤6 BPM,第二组, 7~10 BPM,第三组,>10 BPM.结果 冠状动脉图像质量与心率、心率波动均呈负相关,P<0.001.心率波动1 BPM以下时,欲获得符合诊断要求的冠状动脉图像,允许的最高心率不超过64.5 BPM.统计显示:A、B、C、D各组及第一、二、三组冠状动脉节段显示率总体差别有显著性意义,两两比较,A与B组间差别不明显(P=0.017), A与B组合并后与其它各组间及第一、二、三组各组间差别均显著(P均<0.005).结论 不同心率、心率波动患者,图像质量迥异,欲获得高质量图像心率仍应尽量低、心率波动应尽量小.
Objective To investigate the influence of heart rate and fluctuation of heart rate on image quality of 16-slice spiral CT coronary angiography (16-MSCTA). Methods 16-MSCTA was performed with 0.4s gantry rotation in 89 consecutive patients with different heart rate and different fluctuation of heart rate. According to the heart rate, the patients were classified into 4 groups: group A, the heart rate〈60 beat per minute (BPM); group B, 60-69 BPM; group C, 70-79 BPM; group D, ≥80 BPM. According to the fluctuation of heart rate, these patients were classified into 3 groups: the first group, its fluctuation of heart rate≤6 BPM; the second group, 7-10 BPM; the third group, 〉10 BPM. Results Significant negative correlations were observed both between heart rate and image quality and between fluctuation of heart rate and image quality (P〈0. 001). To achieve high image quality, the heart rate should less than 64. 5 BPM, if the fluctuation of heart rate was less than 1 BPM. Statistical analysis showed: there was totally significant difference in the rate of assessable coronary segments among group A, B, C, D and the first, the second, the third group. Comparing between every two groups, there was no significant difference between group A and B (P〈0. 017), so the two groups were combined to form group A+B, as result there were significant differences between every two groups in A+B, C, D and in the first, the second, the third group (P〈0. 005). Conclusion The image quality was significantly different in patients with different heart rate and different fluctuation of heart rate. To achieve high image quality, the heart rate should be sufficiently slow and the fluctuation of heart rate should be sufficiently fow.
