摘要
目的探讨促排卵过程中卵泡发育速度与卵母细胞质量间的相关性。方法154名患者共159个体外受精-胚胎移植(IVF-ET)周期,按卵泡发育速度(v,mm/d)的分布分为A组(v<1)、B组(1≤v≤1.59)、C组(v>1.59),比较各组卵母细胞成熟度指标。结果A、B、C3组卵细胞成熟率分别为89.56%、92.45%和94.76%,优质胚胎率分别为40.85%、36.37%和37.11%,妊娠率分别为42.1%、43.1%和35.3%,卵母细胞成熟率C组高于A、B组,优质胚胎率A组高于B、C两组,但组间差异均无显著性(P>0.05),而妊娠率A、B两组明显高于C组,且B、C两组间差异有显著性(P<0.05)。结论促排卵过程中卵泡的生长速度应控制在一个最佳的范围内,因其与卵子质量和后期优质胚胎率及妊娠结局密切相关,且卵泡发育速度又可作为衡量卵泡发育潜能的一项重要指标。
Objective To investigate the relationship between follicular growth velocity and oocyte quality of ovulation induction. Methods According to the follicular growth velocity(v,mm/d), 154 patients, 159 IVF-ET cycles ,which causes of infertility included unexplained infertility, tubal and male factor were divided into three groups: A (v 〈 1 ), B ( 1≤ v ≤1.59 ), C (v 〉 1.59). Then we calculated the oocyte maturing rate, high-quality embryo rate pregnancy outcome in these groups respectively, and analyzed if there was significant deviation among the three groups. Results In group A, B and C, the oocyte maturing rates were 89.56% ,92.45% ,94.76% ; highquality embryo rates were 40. 85%, 36. 37%, 37.11% ;and pregnancy rates were 42. 1%, 43.1% , 35.3%. The maturing rate of group C was higher than that of the others. The high-quaihy embryo rate of group A was higher than that of the others, but there were no significant deviation among those groups(P 〉0. 05). The pregnancy rate of group A and B was obviously higher than that of group C, furthermore, there were significant deviation between group B and C. Conclusion There is a close relationship between follicular growth velocity of the ovulation induction and oocyte quality, high-quality embryo rate and pregnancy outcome in the later stage. Follicular growth velocity can be seen one of the important indicators for measuring the potential of follicular development.
