区域淋巴组织靶向化疗诱导兔乳腺移植性鳞癌模型淋巴结转移癌细胞凋亡

摘要 目的:研究区域淋巴组织靶向化疗是否能有效诱导兔乳腺移植性鳞癌模型腋窝淋巴结转移癌细胞凋亡,并与常规静脉化疗作比较。方法:30只乳腺种植VX2肿瘤的新西兰兔随机分为3组,每组10只。腋淋巴结最大直径达到5mm时开始治疗。淋巴化疗组:肿瘤周围皮下注射多柔比星脂质体(LADR);静脉化疗组:经耳缘静脉注射多柔比星水溶剂(FADR);对照组:皮下注射生理盐水。治疗组单次给药剂量均为1mg/kg,每48小时重复给药1次,共给药3次。第3次治疗后48小时切除腋淋巴结,测量治疗前后淋巴结体积,TUNEL检测转移灶癌细胞凋亡指数(AI),ANOVALSD多重比较分析组间差异。结果:与对照组相比,治疗组均出现淋巴结增长减慢,淋巴化疗组的抑制作用又强于静脉化疗组(P=0.002)。淋巴化疗、静脉化疗组均可见明显的转移癌细胞凋亡,平均AI分别为21.73%、15.31%,与对照组(5.16%)相比均有显著差异(P=0.000),淋巴化疗组又显著高于静脉化疗组(P=0.000)。结论:与静脉化疗相比,区域淋巴组织靶向化疗能更加有效地诱导乳腺癌淋巴转移灶的肿瘤细胞凋亡。 Purpose:To investigate whether chemotherapy targeting the regional lymphatic tissues could induce apoptosis of metastatic tumor cells in axillary lymph nodes in rabbits bearing VX2 squamous cell carcinoma in the mammary gland. Methods:Thirty female New Zealand rabbits with mammary implantation of VX2 tumors were divided into 3 groups randomly, with 10 in each. Treatment was initiated once the maximum diameter of axillary lymph node attained 5 mm. Group A received a subcutaneous injection of liposomal adriamycin (LADR) adjacent to the induced tumor. Group B received free adriamycin (FADR) injected into the auricular vein. Group C received a placebo treatment. The dose of adriamycin in each administration was 1mg/kg in groups A and B. Treatment was repeated every 48 hours. Axillary lymph nodes were resected 48 hours after the third treatment. The nodal sizes before and after treatment were measured. Apoptosis of metastatic cells in lymph nodes was identified and quantified as apoptosis index (AI) using the terminal deoxynucleotidyl transferase-mediated dTUP nick end labeling (TUNEL) technique. The significance of the differences was determined with the one-way ANOVA followed by the Fischer LSD test. Results:Compared with group C, the enlargement of lymph node was slowed down in both groups A and B. Group A showed a more marked inhibition on nodal growth than group B (P=(0.002)).Marked apoptosis of metastatic tumor cells in nodes were detected in both groups A and B. The mean AIs in groups A, B and C were 21.73, 15.31 and 5.16%, respectively, with significant differences between group A and group B or C (P=0.000 for both), as well as between groups B and C (P=0.000). Conclusions:Apoptosis of metastatic tumor cells in regional lymph nodes can be more effectively induced by lymphatic targeting chemotherapy in comparison with by intravenous chemotherapy.
