摘要
目的 :探讨比较单药长春瑞滨 (VNB) +放疗 (RT)与VNB +顺铂 (DDP)联合化疗治疗对老年人晚期非小细胞肺癌 (NSCLC)的疗效及安全性。方法 :5 5例随机分两组 ,VNB +DDP组 2 7例 ,VNB 2 5mg/m2 ,d1、5 ,DDP 2 5mg～ 30mg/m2 d2、3、4。每 2 1天重复。VNB +RT组 2 8例 ,VNB 2 5mg/m2 ,d1、8、2 9、36天 ,第 2天开始放疗 ,DT 5 5～ 6 5Gy。 结果 :近期有效率 :VNB +DDP组 4 4 4 % ,VNB +RT组 5 3 6 % (P <0 0 5 )。两组中位生存期分别为 2 9 2个月和 11 6个月 ,1、2、3年生存期VNB +DDP组为 4 9 2 %、2 0 1%和 6 8% ,VNB +RT组为 5 1 6 %、2 2 5 %和 7 4 % (P <0 0 5 )。毒副反应方面两组有明显差异 (P <0 0 5 )。结论 :单药VNB +放疗治疗老年人晚期NSCLC安全有效 ,具有良好耐受性。
Objective:To evaluate and compare the effects and safety of VNB+radiotherapy(RT) regimen and VNB+DDP regimen for patients with old age advanced non small cell lung cancer(NSCLC).Methods:55patients with old age advanced NSCLC were randomized into two groups. There are 27 patients in VNB+DDP group, VNB 25mg/m2 d1,5, DDP 25mg～30mg/m2 d2,3,4. Repeated every 21 days. There are 28 patients in VNB+RT group, VNB 25mg/m2, d1,8,29,36, radiotherapy was started on second day, DT 55～65Gy.Results:The short time response rate of VNB+DDP was 44.4% and VNB+RT was 53.6% (P<0.05), the median survival time(MST) and 1 year,2 year,3 year survival rate were as follow: VNB+DDP group:9.2 months and 49.2%, 20.1%, 6.8%. VNB+RT group: 11.6 months and 51.6%,22.5%,7.4%(P<0. 5). In the toxicity, there was statisical significant difference between them (P<0.05).Conclusion:Vinorelbine combined with radiotherapy in the treatment of old age advanced NSCLC was effective and safe, the toxicities was well tolerated.
