谐振频率法分析骨挤压术对种植体初期稳定性的影响 被引量:7

摘要 目的:采用Osstell谐振频率分析仪初步评估骨挤压术对Branemark和Replace种植体稳定性的影响,探讨不同外科技术在提高种植体成功率中的作用。方法:选取上颌后牙Ⅳ类骨质的牙列缺损患者,按照Noblebiocare标准守则,分别采用骨挤压术和传统备洞技术植入Replace和Branemark种植系统共计48颗种植体,其规格均为φ5.0×L10.0mm。记录种植体系统类型、植入最终扭矩和Osstell谐振频率分析仪测定的初期稳定性ISQ值,对观察结果采用双因素方差分析。结果:骨挤压术较传统备洞技术所获得的种植体稳定性高(p<0.05);Replace和Branemark两种系统的种植体稳定性之间也存在有统计学意义的差异,Replace种植系统的初期稳定性较Branemark系统大(p<0.05)。结论:在上颌后牙Ⅳ类骨区,应用谐振频率法能初步定量反映出骨挤压术后种植体稳定性的变化,与传统备洞技术相比较,采用骨挤压术可获得较高的种植体初期稳定性。 Objectives:The objective of this study was set to evaluate the effects of osteotome technique on implant initial stability using resonance frequency analysis,and try to find out the correlation factors that affecting the implant stability.Material and methods:Lekholm & Zarb classification type IV patients was chosen according to Noblebiocare standard,forty-eight φ5.0×L10.0mm Branemark and Replace implants were placed by osteotome technique and conventional preparation technique separately.The initial ISQ value,type of implant and final torque of implant placement were recorded.Double factor analysis of variance was used to compare the initial stability of the two methods and implant systems.Results:Using resonance frequency analysis,implant placed with osteotome technique possess significantly higher stability than those placed with conventional implant placement(p<0.05).There was significantly different stability between Branemark and Replace implants,the Replace implant has a higher initial stability(p<0.05).Conclusions:In the posterior part of maxilla with class IV Lekholm and Zarb classification,the resonance frequency analysis can primarily estimate the quantitative change of implant stability after applying the osteotome technique.The osteotome technique can promote the implant initial stability.
