摘要
目的探讨高尿酸血症患者血清炎症介质超灵敏C反应蛋白(hs-CRP)水平与机体胰岛素敏感性、胰岛素分泌功能的关系。方法运用高灵敏酶联免疫吸附法检测41例高尿酸血症患者和20名健康对照者的血清hs-CRP水平,同时检测血糖和血清胰岛素水平,并计算稳态模型胰岛素抵抗指数(HOMA IR)和胰岛B细胞分泌指数(HOMA IS)以反映胰岛素敏感性和胰岛细胞功能。结果高尿酸血症患者的血清hs-CRP水平(2.18±0.95 mg/L vs 0.89±0.45mg/L,P<0.01)和HOMA IR指数(1.61±0.18 vs 0.92±0.09,P<0.01)显著高于正常对照组,HOMA IS指数低于正常对照组(85.1±14.9 vs 127.6±9.8,P<0.01)。进一步将患者分为合并痛风组和无症状高尿酸血症组后发现,痛风组的血清hs-CRP水平显著高于无症状高尿酸血症组(2.82±1.09 mg/L vs 1.77±0.87 mg/L,P<0.01)。线性相关分析显示,高尿酸血症患者的血清hs-CRP水平与体重指数(BMI)(r=0.44,P<0.01)、血尿酸(r=0.49,P<0.01)2、4 h尿尿酸(r=0.37,P<0.01)和HOMA IR指数(r=0.48,P<0.01)呈显著正相关。结论高尿酸血症患者血清hs-CRP水平高于正常人,并与其胰岛素抵抗的发生、发展有密切的关系。
Objective To investigate the relationship between serum high sensitive C-reactive protein(hs-CRP) levels and insulin sensitivity or islet function in patients with hyperuricemia.Methods Serum hs-CRP level was determined by using highly sensitive enzyme-link immune absorption assay(ELISA) method in 41 patients with hyperuricemia and 20 normal controls.Plasma glucose and serum insulin were also measured. Moreover,homeostasis model analysis(HOMA) insulin resistance(HOMA IR) index and HOMA-islet secretion(HOMA IS) index were calculated to estimate insulin sensitivity and islet function.Results In the patients with hyperuricemia,serum hs-CRP and HOMA IR index were increased,and HOMA IS index decreased as compared with normal subjects.Moreover,serum hs-CRP was higher in hyperuricemia patients with gout than that in asymptomatic hyperuricemia patients.Liner regression analysis in hyperuricemia patients revealed that serum hs-CRP was positively correlated with body mass index,HOMA IR index,serum uric acid and urine uric acid level,respectively.Conclusion Serum hs-CRP level was higher in the patients with hyperuricemia than that in normal subjects,and played a role in the onset and development of insulin resistance.
