早期肠内营养对腹腔镜结直肠癌根治术后患者胃肠功能恢复及肠黏膜屏障功能的影响 被引量:1

Effects of Early Enteral Nutrition on Gastrointestinal Function Recovery and Intestinal Mucosal Barrier Function in Patients Undergoing Laparoscopic Radical Resection of Colorectal Cancer

摘要 目的探讨早期肠内营养对腹腔镜结直肠癌根治术后患者胃肠功能恢复及肠黏膜屏障功能的影响.方法选择该院2018年7月—2021年1月收治的69例腹腔镜结直肠癌根治术患者为研究对象,按照术后营养干预方式分为对照组(n=34)和研究组(n=35).对照组采取早期肠外营养支持,研究组采取早期肠内营养支持.观察比较两组的胃肠功能恢复情况、营养指标状况、肠黏膜屏障功能指标水平及并发症发生情况.结果研究组的肠鸣音恢复时间、首次排气时间及首次排便时间均明显低于对照组,组间差异有统计学意义(P<0.05).干预后,研究组的转铁蛋白、血清白蛋白及前白蛋白水平均明显高于对照组,组间差异有统计学意义(P<0.05).干预后,研究组的二胺氧化酶、D-乳酸水平及尿中乳果糖与甘露醇比值均明显低于对照组,组间差异有统计学意义(P<0.05).研究组的并发症总发生率为14.29%,明显低于对照组的38.24%,差异有统计学意义(P<0.05).结论早期肠内营养支持能够促进腹腔镜结直肠癌根治术后患者的胃肠功能恢复,改善患者的营养指标,保护肠黏膜屏障功能,减少术后并发症,值得临床推广使用. Objective To explore the effect of early enteral nutrition on the recovery of gastrointestinal function and intestinal mucosal barrier function in patients after laparoscopic radical resection of colorectal cancer.Methods The 69 patients undergoing laparoscopic radical resection of colorectal cancer who were admitted to the hospital from July 2018 to January 2021 were selected as the research subjects,and were divided into control group(n=34)and study group(n=35)according to postoperative nutritional intervention methods.The control group adopted early parenteral nutrition support,and the study group adopted early enteral nutrition support.Observation and comparison of the recovery of gastrointestinal function,nutritional indicators,intestinal mucosal barrier function indicators and complications of the two groups.Results The recovery time of bowel sounds,the time of first exhausting and the time of first defecation in the study group were significantly lower than those of the control group,and the difference between the groups was statistically significant(P<0.05).After the intervention,the transferrin,serum albumin and prealbumin levels of the study group were significantly higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).After the intervention,the levels of diamine oxidase,D-lactic acid and the ratio of lactulose to mannitol in the study group were significantly lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).The total incidence of complications in the study group was 14.29%,which was significantly lower than 38.24%in the control group,and the difference was statistically significant(P<0.05).Conclusion Early enteral nutrition support can promote the recovery of gastrointestinal function in patients after laparoscopic radical resection of colorectal cancer,improve the nutritional indicators of patients,protect the intestinal mucosal barrier function,and reduce postoperative complications.It is worthy of clinical application.
