早期高压氧联合常规康复锻炼对高血压脑出血患者术后康复效果的影响 被引量:3

Effect of Early Hyperbaric Oxygen Combined with Routine Rehabilitation Exercise on the Rehabilitation Effect in Patients with Hypertensive Intracerebral Hemorrhage after Operation

摘要 目的探讨早期高压氧联合常规康复锻炼在高血压脑出血(HICH)术后患者中的应用效果.方法选择2019年10月—2020年12月该院收治的HICH患者88例,按随机数字表法分为两组,各44例.两组患者均行手术治疗,对照组术后实施常规康复锻炼,观察组术后在对照组基础上采用早期高压氧,两组均持续观察至术后2个月.比较两组脑水肿体积、水通道蛋白-4(AQP4)水平、炎症因子水平、美国国立卫生研究院卒中量表(NIHSS)评分、日常生活能力(ADL)评分、并发症发生率.结果观察组干预后脑水肿体积小于对照组,AQP4水平、肿瘤坏死因子-α、白细胞介素-8、C反应蛋白水平、NIHSS评分均低于对照组,ADL评分高于对照组,组间差异有统计学意义(P<0.05);观察组并发症发生率4.55%(2/44)低于对照组的18.18%(8/44),差异有统计学意义(P<0.05).结论早期高压氧联合常规康复锻炼在HICH术后患者中具有较好的应用效果,利于改善脑水肿情况,减轻炎症反应,促进神经功能与日常生活能力的恢复,降低并发症发生率. Objective To explore the application effect of early hyperbaric oxygen combined with routine rehabilitation exercise in patients with hypertensive cerebral hemorrhage(HICH).Methods 88 patients with HICH admitted to the hospital from October 2019 to December 2020 were divided into two groups according to the random number table method,with 44 patients in each group.The patients in both groups were treated with surgery,the control group was treated with routine rehabilitation exercise after operation,and the observation group was treated with early hyperbaric oxygen on the basis of the control group after operation.Both groups were continuously observed until 2 months after operation.The volume of brain edema,aquaporin-4(AQP4)level,inflammatory factors levels,National Institutes of Health stroke scale(NIHSS)score,activity of daily living scale(ADL)score and incidence of complications were compared between the two groups.Results After intervention,the volume of brain edema in the observation group was smaller than that in the control group,AQP4 level,tumor necrosis factor-α,interleukin-8,C-reactive protein levels and NIHSS score were lower than those in the control group,and the ADL score was higher than that in the control group,with statistically significant differences between the groups(P<0.05);the incidence of complications 4.55%(2/44)in the observation group was lower than 18.18%(8/44)in the control group,with statistically significant difference(P<0.05).Conclusion The early hyperbaric oxygen combined with routine rehabilitation exercise has a good application effect in patients after HICH,which is conducive to improving brain edema,reducing inflammatory reaction,promoting the recovery of neural function and daily living ability,and reducing the incidence of complications.
