摘要
目的探讨损伤控制(DCS)联合快速康复外科(FTS)理念在严重肺挫伤并发肋骨骨折患者救治中的运用效果.方法回顾性分析2020年2月-2022年2月本院收治的84例严重肺挫伤并发肋骨骨折患者的临床资料,将采用常规救治的42例纳入对照组,将在救治过程中应用DCS联合FTS理念的42例纳入观察组.对比两组的机体恢复情况、并发症发生率、急性生理和慢性健康状况(APACHEⅡ)评分.结果观察组术后72 h的疼痛视觉模拟评分为(2.01±0.35)分,低于对照组,机械通气时间、胸管留置时间、ICU住院时间、总住院时间分别为(0.58±0.16)d、(2.13±0.38)d、(3.24±0.78)d、(13.29±2.11)d,均短于对照组,组间差异有统计学意义(P<0.05).观察组的并发症发生率为2.50%,低于对照组的19.05%,差异有统计学意义(P<0.05).观察组治疗后的APACHEⅡ评分为(6.12±1.07)分,低于对照组,差异有统计学意义(P<0.05).结论DCS联合FTS理念的应用能提升严重肺挫伤并发肋骨骨折患者的救治效果,减少并发症的发生,改善机体健康水平,促进术后恢复.
Objective To explore the effect of damage control surgery(DCS)combined with fast track surgery(FTS)in the treatment of severe pulmonary contusion complicated with rib fracture.Methods The clinical data of 84 patients with severe pulmonary contusion complicated with rib fracture admitted to our hospital from February 2020 to February 2022 were retrospectively analyzed.42 cases of routine treatment were included in the control group,and 42 cases applied DCS combined with FTS concept in the treatment process were included in the observation group.The body recovery,complication rate,acute physiology and chronic health evaluation II(APACHEⅡ)score were compared between the two groups.Results At 72 hours after operation,the visual analogue score of pain in the observation group was(2.01±0.35)points,which was lower than that in the control group,and the mechanical ventilation time,chest tube induration time,ICU stay time and total hospital stay were(0.58±0.16)d,(2.13±0.38)d,(3.24±0.78)d and(13.29±2.11)d,respectively,which were shorter than those of the control group,and the differences between the two groups were statistically significant(P<0.05).The complication rate of the observation group was 2.50%,which was lower than 19.05%of the control group,and the difference was statistically significant(P<0.05).The APACHEⅡscore of the observation group after treatment was(6.12±1.07)points,which was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of DCS combined with FTS concept can improve the treatment effect of severe pulmonary contusion complicated with rib fracture,reduce the incidence of complications,improve the body health level,and promote postoperative recovery.
